摘要
目的:探究中山市RhD阴性血型血清学和基因分型特征。方法:选择2017年12月—2019年2月中山市我院门诊和住院患者,以微柱凝胶血型卡进行RhD抗原筛查24286人次,阴性者以抗人球蛋白卡(IAT)对初筛者进行确认,同时采用RhCcEc分型血清进行RhD阴性表型分型,采用PCR-SSR法进行RhD阴性基因分型。结果:①24286例筛查中RhD阴性者102例,占总数的0.42%,其中A型24.51%,B型30.39%,O型39.22%,AB型5.88%。②经确认,102例RhD阴性者中95例为阴性,7例为IAT阳性。③RhD阴性者血清学表型主要为ccee(46.08%)和Ccee(36.27%)。④RhD阴性基因分型主要为RHD全缺失型(56.86%)和DEL RhD1277A型(31.37%)。结论:对中山市RhD阴性血型进行血清学和基因分析,可以更准确地为本地RhD血型定型,促进中山市RhD阴性资料库的建立,为提高RhD阴性患者临床用血安全,预防自身溶血病及RhD新生儿溶血病提供科学依据。
Objective:To explore the serological and genotyping characteristics of RHD negative blood groups in Zhongshan city.Methods:Choose in December 2017 to February 2019 in zhongshan hospital outpatient and inpatient,RhD antigen screening in micro column gel type card 24286 person-time,negatie with anti globulin CARDS(IAT)for early screening for confirmation,at the same time with RhCcEc parting serum RhD negative phenotype classification,by PCR,SSR method RhD negative genotyping.Results:①102 cases(0.42%)of 24286 screening cases were RHD negative,including 24.51%of type A,30.39%of type B,39.22%of type O and 5.88%of type AB.②Of the 102 patients with RhD negative,95 were negative and 7 were IAT positive.③The serological phenotypes of RhD negative were mainly CCEE(46.08%)and CCEE(36.27%).④RHD negative genotypes were mainly RHD complete deletion type(56.86%)and Del RHD 1277A type(31.37%).Conclusion:Serological and genetic analysis of RHD-negative blood groups in Zhongshan City can more accurately define the local RHD-negative blood groups,promote the establishment of RHD-negative database in Zhongshan City,and provide scientific basis for improving the safety of clinical blood use of RHD-negative patients,preventing self-hemolytic disease and RHD-newborn hemolytic disease.
