斑秃患者血清25-羟基维生素D测定的意义 被引量:2

摘要 目的:探讨斑秃(AA)患者血清中25-羟基维生素D[25-(OH)D]变化及临床意义。方法:收集50例诊断为斑秃的患者(斑秃组),抽取清晨空腹静脉血,分别检测血清25-(OH)D和C-反应蛋白(CRP)水平;同时收集60例健康人群(对照组)的血清,分别检测CRP和25-(OH)D水平。结果:斑秃组血清25-(OH)D水平(14.17±7.82ng/mL)显著低于对照组(19.24±8.71ng/mL)(P<0.05)。斑秃组中有32例(64.00%)血清25-(OH)D水平低于正常值下限15ng/mL,对照组为41.67%(25/60),2组血清不同浓度25-(OH)D的分布差异有统计学意义(P<0.05)。斑秃组CRP水平与对照组比较差异无统计学意义(P>0.05)。逐步回归分析表明血清25-(OH)D<15 ng/mL为斑秃发病的独立危险因素(P=0.020)。结论:斑秃患者25-(OH)D水平明显低于健康人群,维生素D缺乏可能为斑秃发生的一个重要危险因素。 [Objective]To investigate the serum levels of 25-( OH) D in patients with alopecia areata( AA) and its clinical significance. [Method]Fifty AA patients and sixty healthy controls were enrolled in this study. Mornitoring fasting venous blood samples were collected from AA patients( AA group) and healthy people( control group) to detect serum25-( OH) D and CRP levels. Serum 25-( OH) D and CRP levels were compared between AA patients and healthy controls respectively. Regression analysis was carried out to explore independent correlation of AA. [Result]Serum levels of 25-( OH) D were lower in AA than in control group[( 14. 17 ± 7. 82) ng/mL vs.( 19. 24 ± 8. 71) ng/mL,P <0. 05]. In 64. 00%( 32/50) of AA and 41. 67%( 25/60) of controls,the serum 25-( OH) D levels were below the normal levels of 15 ng/mL although the 25-( OH) D levels were significantly different between AA and controls( P <0. 05). There was no difference in CRP levels between AA and controls. Multiple Logistic regression analysis showed that serum vitamin D levels < 15 ng/mL( OR = 2. 518,95% CI 1. 116 ~ 5. 683,P = 0. 026) was associated with AA.[Conclusion]Serum 25-( OH) D levels in AA patients were significantly lower than that in healthy control. Vitamin D deficiency can be a significant risk factor for AA occurrence.
