慢性肾衰竭尿毒症患者予以高通量血液透析治疗对毒素的清除效果 被引量:2

摘要 目的分析采取高通量血液透析治疗慢性肾衰竭尿毒症患者对毒素的清除效果。方法本次的研究对象从我院在2018年6月至2020年5月收治的慢性肾衰竭尿毒症患者中选取,共60例,均采取血液透析治疗,将其随机分别纳入高通量组和低通量组,两组各30例。对比两组患者在治疗前后的多项毒素指标水平、治疗后的尿素透析充分性以及透析治疗过程中的不良反应发生情况。结果在治疗前,两组患者的PTH、Cystatin C、β2-MG、SCr、BUN、血钾、血磷指标水平均无显著差异(P>0.05);经过治疗后,两组患者的上述指标均有所降低,且高通量组患者的PTH、Cystatin C、β2-MG指标水平低于对照组(P<0.05),而两组在SCr、BUN、血钾、血磷指标水平上无显著差异(P>0.05)。两组患者的尿素清除指数、标准蛋白质分解代谢率以及时间平均尿素浓度均无显著差异(P>0.05)。两组患者在血液透析治疗过程中均存在一定不良反应,但两组间对比无显著差异(P>0.05)。结论采取高通量血液透析治疗慢性肾衰竭尿毒症患者时,优势在于能有效清除小分子毒素的基础上,能够清除更多的大分子毒素,对患者病情的改善具有更显著的效果,存在一定不良反应,临床治疗中应当做好相关预防措施和处理准备。 Objective To analyze the efficacy of high-throughput hemodialysis in the treatment of uremia patients with chronic renal failure.Methods The subjects of this study were selected from 60 patients with chronic renal failure uremia from June 2018 to May 2020.All of them were treated by hemodialysis and randomly included in the high-pass group and the low-flux group.There were 30 cases in each of the two groups.The levels of multiple toxin indicators before and after treatment,the adequacy of urea dialysis after treatment,and the occurrence of adverse reactions during dialysis treatment were compared between the two groups.Results Before treatment,there was no significant difference in the levels of PTH,Cystatin C,β2-MG,SCr,BUN,blood potassium,and blood phosphorus between the two groups of patients (P>0.05);after treatment,the above indicators of the two groups were all The levels of PTH,Cystatin C,and β2-MG in the high flux group were lower than those in the control group (P<0.05),while the two groups had no significant differences in the levels of SCr,BUN,serum potassium,and serum phosphorus (P>0.05).There were no significant differences in the urea clearance index,standard protein catabolism rate,and time-average urea concentration between the two groups (P>0.05).Both groups had some adverse reactions during hemodialysis treatment,but there was no significant difference between the two groups (P>0.05).Conclusion When high-throughput hemodialysis is used to treat uremia patients with chronic renal failure,the advantage lies in the effective removal of small molecular toxins,the removal of more macromolecular toxins,and the improvement of patients’ condition.There are some adverse reactions,clinical treatment should be prepared for relevant preventive measures and treatment.
