摘要
目的探讨两种时机行开腹手术治疗妊娠晚期合并急性阑尾炎的临床效果。方法选取2014年6月至2019年6月收治的妊娠晚期合并急性阑尾炎患者共69例,将24 h内实施开腹手术治疗的36例患者纳入观察组,超过24 h实施开腹手术治疗的33例患者纳入对照组。观察对比2组手术效果、手术相关指标、妊娠结局及并发症情况。结果2组均成功完成手术,未见死亡病例,观察组穿孔率为3%,与对照组18%对比,明显较低(P<0.05);观察组手术时间、术后排气时间、术后下床时间、住院时间均明显短于对照组,术中出血量明显少于对照组(P<0.05);观察组低体质量儿、早产率分别为8%、6%,明显低于对照组27%、24%(P<0.05);2组流产、先天异常率对比,差异无统计学意义(P>0.05);观察组并发症发生率为8%,明显低于对照组30%(P<0.05)。结论妊娠晚期合并急性阑尾炎在两种时机下均可成功完成开腹手术,但24 h内行开腹手术在减少阑尾穿孔,缩短手术时间,促进术后恢复,改善妊娠结局,降低并发症发生风险方面均具有明显优势,更值得推广。
Objective To investigate the clinical effect of open surgery at two timings in the treatment of acute appendicitis in late pregnancy.Methods A total of 69 patients with acute appendicitis in late pregnancy who were treated from Jun 2014 to Jun 2019 were selected,36 patients who underwent open surgery within 24 hours were included in the observation group,and 33 patients who underwent open surgery during 24 hours were included in the control group,the surgical effects,surgical indicators,pregnancy outcomes and complications of the two groups were observed.Results Surgery were successfully completed in both groups,with no deaths observed,the perforation rate in the observation group was 3%,compared with the control group(18%),significantly lower(P<0.05).The observation group's operation time,postoperative exhaust time,postoperative bed time,and hospital stay were significantly shorter than the control group,and the intraoperative blood loss was significantly less than the control group(P<0.05).The low birth weight infants and preterm births in the observation group were 8%and 6%respectively,compared with the 27%and 24%in the control group,significantly lower(P<0.05),there were no significant difference in abortion and congenital abnormalities between the two groups(P>0.05).The incidence of complications in the observation group was 8%,compared with 30%in the control group,significantly lower(P<0.05).Conclusions Patients with acute appendicitis in late pregnancy can successfully complete open surgery at two timings,but open surgery within 24 hours can reduce appendix perforation,shorten the operation time,promote postoperative recovery,improve pregnancy outcomes,and reduce the risk of complications,has obvious advantages,worth promoting.
