摘要
目的:探讨肾血管性高血压(renovascular hypertension,RVH)患者的心功能与肾动脉血流阻力指数(renal resistive index,RRI)的相关性。方法:选取2018年7月-2020年7月于本院诊治的高血压患者160例作为研究对象,根据高血压病因将其分为原发组(原发性高血压患者,n=134)与继发组(RVH患者,n=26)。比较两组的肾功能与心功能指标;比较两组主肾动脉、肾动脉、叶段动脉的RRI并分析与心功能指标的相关性。结果:继发组的血清肌酐(Scr)与尿素氮(BUN)均高于原发组,且肾小球滤过率(eGFR)低于原发组,差异均有统计学意义(P<0.05)。继发组左室舒张末期内径(LVEDd)与左室质量指数(LVMI)均高于原发组(P<0.05);两组左室后壁厚度(LVPWTd)与室间隔厚度(IVSTd)比较,差异均无统计学意义(P>0.05)。继发组主肾动脉、肾动脉、叶段动脉的RRI均高于原发组,差异均有统计学意义(P<0.05)。Pearson相关性检验分析显示,继发组中主肾动脉、肾动脉、叶段动脉的RRI与LVEDd、LVMI均存在正相关性(P<0.05)。结论:相对于原发性高血压,肾血管性高血压患者多伴随有心功能与RRI异常,且两者存在相关性。
Objective:To investigate the correlation between cardiac function and renal radial resistance index(RRI)in patients with renovascular hypertension(RVH).Method:A total of 160 hypertensive patients treated in our hospital from July 2018 to July 2020 were selected as the research subjects.According to the etiology of hypertension,they were divided into the primary group(essential hypertension patients,n=134)and the secondary group(RVH patients,n=26).The indexes of renal function and heart function were compared between the two groups.The RRI of the main renal artery,renal artery and segmental artery were compared between the two groups,and the correlation between RRI and cardiac function indexes was analyzed.Result:The serum creatinine(Scr)and urea nitrogen(BUN)in the secondary group were higher than those in the primary group,and the glomerular filtration rate(eGFR)was lower than that in primary group,the differences were statistically significant(P<0.05).Left ventricular end-diastolic diameter(LVEDd)and left ventricular mass index(LVMI)in secondary group were higher than those in primary group(P<0.05).There was no significant difference in left ventricular posterior wall thickness(LVPWTd)and interventricular septum thickness(IVSTd)between the two groups(P>0.05).The RRI of the main renal artery,renal artery and segmental renal artery in the secondary group were higher than those in the primary group,with statistical significance(P<0.05).Pearson correlation analysis showed that RRI of the main renal artery,renal artery and segmental artery were positively correlated with LVEDd and LVMI in the secondary group(P<0.05).Conclusion:Compared with essential hypertension,patients with renal vascular hypertension are more associated with abnormal cardiac function and RRI,and the two are correlated.
