摘要
目的探讨心脑。肾血管风险评估与高血压前期及其不同血压亚组的关联。方法选取2014年5月至2017年6月医院收治的高血压前期患者90例设为观察组,根据高血压亚型分为单纯收缩期高血压亚型(ISH)、单纯性舒张期高血压亚型(IDH)及联合性高血压亚型(SDH);取同期入院健康体检的血压正常人群90例设为对照组。两组入院后均建立健康档案,进行相应的干预,对两组均进行2年随访,记录两组心脑血管事件发生率及相关性。结果观察组ISH亚型、IDH亚型及SDH亚型中总胆固醇、甘油三酯、低密度脂蛋白胆固醇及血清尿酸水平比较未见统计学差异(P>0.05);观察组总胆固醇、甘油三酯、低密度脂蛋白胆固醇及血清尿酸水平,均高于对照组(P<0.05);观察组90例患者中15例2年后发生心脑肾血管疾病,发生率为16.67%。单因素及Logistic回归分析显示:高血压前期及不同血压亚组心脑肾血管发生率与肥胖、吸烟、高尿酸血症、糖代谢异常及高血压家族史等关系密切(X2=7.443,P<0.05)。结论高血压前期及不同血压亚组与心脑肾血管疾病存在紧密的联系,应采取有效的措施控制患者血压,降低心脑肾血管疾病发生率。
Objective To investigate the association between cardiovascular and brain and renal vascular risk assessment and apprehension and different blood pressure subgroups.Methods From May 2014 to June 2017,90 patients with apprehension were enrolled in this study.The patients were divided into 3groups:the systolic hypertensive subtype (ISH),the simple diastolic Blood pressure subtype (IDH)and joint hypertension (SDH).90normal blood pressure normal subjects admitted to the hospital were selected as the control group.2 groups patients were admitted to hospital after the established the health records,given the corresponding intervention,followed 2 years,recorded the cardiovascular and brain and renal vascular events and the correlation.Results There was no significant difference in total cholesterol,triglyceride,low density lipoprotein cholesterol and serum uric acid between ISH subtype,IDH subtype and SDH subtype (P>0.05).There were 15 cases of heart,brain and renal vascular events within 2 years in the observation group.Uni-variate and Logistic regression analysis showed that the incidence of cardiovascular and brain and renal vascular events in the apprehensive and different blood pressure subgroups was closely related to obesity,smoking,hyperbaric, abnormal glucose metabolism and family history of hypertension (x2=7.443,P<0.05).Group of 90 patients in 15 cases 2 years after the occurrence of heart and brain renal vascular disease,the incidence was 16.67%.The incidence of cardiovascular and vascular diseases was significantly higher than that of the control group (P<0.05). Conclusion Apprehension and different blood pressure subgroups are closely related to cardiovascular and brain and renal vascular diseases.Effective measures should be taken to control blood pressure and reduce the incidence of cardiovascular and brain and renal vascular diseases.
