摘要
“新病入络”具有邪气初期侵袭体表阳络,迅速传入脏腑阴络,病情急骤的特点。纵观新冠肺炎的临床表现和病理演变过程,基本符合“新病入络”的病机发展规律。疫毒之性暴烈是新冠疫气“新病入络”发生的前提条件;“直中肺络,表里两感”是发病的首要特点;湿、毒、燥、痰、瘀等邪气痹阻肺络是必经阶段;从气到血是发生脏腑传变的主要途径;络气虚滞,气阴两虚是病机发展的必然转归。故临床以综合辨证,防变防传为治疗原则,强调在分期论治,络为核心的基础上,尽早治络,截断传变;表里同治,奏效乃捷。
Acute diseases entering collaterals,which means that evil-Qi attacks the Yang collaterals on the body surface at the early stage,then rapidly enters into the viscera and Yin collaterals,leading to acute illness.According to the clinical manifestations and pathological changes of COVID-19,it basically conforms to the law of pathogenesis of‘acute diseases entering the collaterals’.The violent nature of pestilential pathogen is the prerequisite for the occurrence of COVID-19 in terms of‘acute disease entering collaterals’.‘Direct attacking Lung,invading exterior and interior’is the prime feature of the onset of the disease.Dampness,toxin,dryness,phlegm and blood stasis blocking Lung collaterals is the necessary stage of‘acute diseases entering collaterals’.Pathological change from Qi to blood is the main way of COVID-19 transmission between Zang and Fu.Deficiency of Qi and Yin,as well as deficiency of collaterals-Qi and loss of nourishment are the inevitable outcome of the development of pathogenesis.Therefore,the treatment principle for COVID-19 is to differentiate syndrome comprehensively and prevent changes and transmission of the disease.It emphasizes the treatment based on the stages,taking collaterals as the core and to treat them as early as possible to cut off the transmission.Treating both exterior and interior is effective for COVID-19.
