摘要
目的探讨芪苈强心胶囊和胺碘酮治疗老年心力衰竭合并阵发性房颤的价值研究。方法将2018年1月—2020年6月在该院心内科治疗的150例老年心力衰竭合并阵发性房颤患者随机分为两组,每组各75例,均采用常规西药抗心衰治疗,在此基础上,对照组使用胺碘酮治疗,观察组使用芪苈强心胶囊和胺碘酮治疗,对比两组的临床疗效、心功能指标变化、NT-proBNP及QTd变化、中医证候积分变化。结果观察组治疗有效率为94.67%(71/74),明显高于对照组的78.67%(59/71)(P<0.05);观察组治疗后LVEF、CO明显高于对照组,而LVESD、LVEDD、LVMI、心率明显小与对照组(P<0.05);观察组治疗后NT-proBNP水平明显低于对照组,QTd明显小于对照组(P<0.05);观察组治疗后心悸气短、倦怠乏力、下肢水肿、畏寒肢冷等中医证候积分明显低于对照组(P<0.05)。结论芪苈强心胶囊和胺碘酮治疗老年心力衰竭合并阵发性房颤的价值显著,能有效增强心肌收缩力,抑制心室重构,减轻心肌损伤程度,抑制房颤发作,保护心功能,具有积极的临床意义。
Objective To investigate the value of Qili Qiangxin Capsule(芪苈强心胶囊) and amiodarone in the treatment of senile heart failure complicated with paroxysmal atrial fibrillation. Methods A total of 150 elderly patients with heart failure complicated with paroxysmal atrial fibrillation in our hospital from January 2018 to June 2020 were randomly divided into two groups, 75 cases in each group. On this basis, the control group was treated with amiodarone, and the observation group was treated with Qili QiangxinCapsule and amiodarone. The changes of NT proBNP, QTd and TCM syndrome score were observed. Results The effective rate of the observation group was 94.67%(71/74), which was significantly higher than 78.67%(59/71)of the control group(P<0.05). LVEF and CO in the observation group were significantly higher than those in the control group, while the levels of LVESD, LVEDd and LVMI and heart rate in the observation group were significantly lower than those in the control group(P<0.05). After treatment, the level of NT proBNP in the observation group was significantly lower than that in the control group, and QTd was significantly lower than that in the control group(P<0.05). The observation group had palpitations, shortness of breath and fatigue after treatment. The scores of traditional Chinese medicine syndromes such as fatigue, edema of lower limbs and chilly limbs were significantly lower than those of the control group(P<0.05). Conclusion Qili Qiangxin Capsule and amiodarone have significant value in the treatment of senile heart failure complicated with paroxysmal atrial fibrillation, which can effectively enhance myocardial contractility, inhibit ventricular remodeling, reduce the degree of myocardial injury, inhibit the onset of atrial fibrillation, and protect cardiac function.
