摘要
目的探讨不同预后的冠心病心力衰竭患者经皮冠状动脉内介入治疗(PCI)术前动态心电图特征。方法回顾性分析接受PCI治疗的60例冠心病心力衰竭患者的临床资料,全部患者均在接受PCI术治疗前行动态心电图检查,依据随访过程中患者是否出现不良心血管事件,将其分为预后良好组(n=50)与预后不良组(n=10)。比较2组手术治疗前动态心电图特征,并绘制ROC曲线,分析动态心电图对患者预后的预测价值。结果本组冠心病心力衰竭患者不良心血管事件发生率为16.67%;预后不良组SDNN、24 h内每5 min一般正常RR期间的SDNNI、SDANN以及rMSSD均低于预后良好组,差异有统计学意义(P<0.05);SDANN与rMSSD预测冠心病心力衰竭患者PCI术后预后的价值高于SDNN与SDNNI。结论冠心病心力衰竭患者PCI术前动态心电图可对预后起到一定的预测作用。
Objective To explore the characteristics of dynamic ECG before PCI in patients with coronary heart disease and heart failure with different prognosis.Methods The clinical data of 60 patients with coronary heart disease and heart failure who received PCI were retrospectively analyzed.All patients underwent Holter examination before PCI treatment.According to whether the patients had adverse cardiovascular events during the follow-up,they were divided into Good prognosis group(n=50)and poor prognosis group(n=10).Compare the baseline data of patients with different prognosis and the characteristics of the ambulatory electrocardiogram before surgery,draw the ROC curve,and analyze the predictive value of the ambulatory electrocardiogram for the patient's prognosis.Results The incidence of adverse cardiovascular events in patients with coronary heart disease and heart failure was 16.67%;the poor prognosis group(SDNN,SDNNI,SDANN and rMSSD during normal RR every 5 minutes for 24 h)were lower than those in the good prognosis group,and the difference was statistical Significance(P<0.05);SDANN and rMSSD are more valuable than SDNN and SDNNI in predicting the prognosis of patients with coronary heart disease and heart failure after PCI.Conclusion In patients with coronary heart disease and heart failure before PCI,the ambulatory ECG can play a certain role in predicting the prognosis of the patients.Therefore,the Holter examination before PCI can provide a reference for prognostic evaluation.
