摘要
目的探讨超声心动图与动态心电图诊断冠心病合并心力衰竭的价值.方法选取60例冠心病合并心力衰竭患者作为研究组,同时选取60例单纯冠心病患者作为对照组.全部患者均采用超声心动图与动态心电图检查.结果研究组LVESD、LVEF、LVFS、SV、E/A与ET均明显低于对照组,LVEDD、ICT、IRT均明显高于对照组,2组比较差异具有统计学意义(P<0.05);研究组SDNN、SDANN、r MSSD均明显低于对照组,研究组HR明显高于对照组,2组比较差异具有统计学意义(P<0.05),研究组ST-T波改变、T波低平或平坦所占比例明显高于对照组(P<0.05);以冠脉血管造影(CAG)结果作为金标准,设定LVEF<50%作为超声心动图诊断阳性标准,设定SDNN<100 ms作为动态心电图的诊断阳性标准,2种检查方法在诊断灵敏度、特异度与准确度的比较差异无统计学意义(P>0.05).结论 LVESD、LVEDD、LVEF、SV、E/A、ET等超声心动图参数与SDNN、SDANN、r MSSD等心电图参数在诊断冠心病合并心力衰竭中均具有重要的价值.
Objective To study the value of ultrasonic cardiogram and dynamic electrocardiogram in diagnosis of coronary heart disease with heart failure. Methods 60 cases of patients with coronary heart disease with heart failure were selected as study group, and another 60 cases of patients with coronary heart disease were selected as control group. All patients were given echocardiography and dynamic electrocardiogram examination. Results LVESD, LVEF, LVFS, SV, E/A and ET of study group were significantly lower than those of control group, LVEDD, ICT, IRT significantly higher than those of control group, and the differences had statistically significant difference （P 〈 0.05） . SDNN, SDANN rMSSD of study group were significantly lower than control group, HR in study group were significantly higher than control group and the differences were statistically significant （P 〈 0.05） . ST - T wave changes, T wave low proportion of study group were significantly higher than those of the control group （P 〈 0.05） With CAG results as the gold standard, LVEF 〈 50% as echocardiography positive diagnosis standard, and SDNN〈100 ms as dynamic electrocardiogram positive diagnosis standard, difference between the two methods in terms of sensitivity, specific and accuracy had no statistically significance （P 〉 0.05） .Conclusion Ultrasonic cardiogram parameters such as LVESD, LVEDD, LVEF, SV, E/A, ET and echocardiographvparameters such as SDNN, SDANN rMSSD are of great importance in the diagnosis of coronary heart disease with heart failure.
