miR-9和MMP-14在非小细胞肺癌中的表达及临床意义 被引量:6

摘要 目的检测微小RNA-9(miR-9)和基质金属蛋白酶-14(MMP-14)在晚期非小细胞肺癌(NSCLC)患者血清和组织中的表达情况,以及与患者临床特征及预后的关系。方法收集76例晚期(Ⅲb~Ⅳ期)NSCLC患者的血液样本和肿瘤组织样本,另收集气管肺部良性疾病患者50例。采用实时荧光定量PCR(qPCR)法检测血清和组织中miR-9表达。采用ELISA法和免疫组化法检测血清和组织中MMP-14表达。随访12个月患者生存情况。采用受试者工作特征(ROC)曲线计算血清miR-9、MMP-14对晚期NSCLC患者预后的预测价值。结果与良性疾病组患者比较,NSCLC组患者血清miR-9水平降低,MMP-14水平升高(P<0.05)。NSCLC患者血清miR-9和MMP-14水平呈负相关性(r=-0.399,P<0.001)。肿瘤组织中MMP-14蛋白阳性表达率为65.79%(50/76),MMP-14阳性表达者组织miR-9表达量低于MMP-14阴性表达患者(P<0.05)。血清和组织中miR-9、MMP-14表达与肺外转移、TNM分期和ALK基因突变有关。血清miR-9和MMP-14水平预测患者1年内死亡的ROC曲线下面积(AUC)分别为0.696(95%CI:0.493~0.823)、0.704(95%CI:0.545~0.798),敏感度和特异度分别为93.3%、52.5%和80.3%、58.5%,截断值分别为0.825和44.80 pg/ml。结论晚期NSCLC患者血清和组织中miR-9表达量降低以及MMP-14水平升高与患者Ⅳ期、发生肺外转移和患者死亡有关,血清miR-9和MMP-14水平对于预测晚期NSCLC患者预后不良有一定的临床参考价值。 Objective To detect the expressions of microRNA-9(miR-9)and matrix metallopeptidase-14(MMP-14)in plasma or tumor tissues of advanced non-small cell lung cancer(NSCLC)patients and to investigate the correlation between miR-9,MMP-14 and clinicopathological characteristics or prognosis.Methods A total of 76 advanced NSCLC patients and 50 patients with pulmonary benign diseases were enrolled into this study,the clinicopathological data and plasma and tumor tissue samples were collected.The death during followed-up was recorded.The expressions of miR-9 were detected by qRT-PCR method,while ELISA and IHC method were used for detection of MMP-14 protein in plasma and tissues.Their relationships with clinicopathological characteristics were analyzed.The diagnostic value of serum miR-9 and MMP-14 on prognosis of advanced NSCLC was analyzed by receiver operating characteristic(ROC)curve.Results Compared with patients with pulmonary benign diseases,serum miR-9 expression in NSCLC patients was lower and MMP-14 level was higher(P<0.05).And there was negative relationship between serum miR-9 and MMP-14(r=-0.399,P<0.001).The positive expression rate of MMP-14 in tumor tissues of NSCLC patients was 65.79%(50/76),and miR-9 expression in patients with positive MMP-14 tissues was lower than that in negative MMP-14 tissues(P<0.05).The expression of miR-9 and MMP-14 in serum and tissues was associated with extrapulmonary metastasis,TNM staging and ALK gene mutation.The areas under predictive ROC curve of serum miR-9 and MMP-14 were 0.696(95%CI:0.493-0.823),0.704(95%CI:0.545-0.798),the sensitivity and specificity values were 93.3%,52.5%and 80.3%,58.5%;and the cut-off values were 0.825 and 44.80 pg/ml.Conclusion In advanced NSCLC patients,miR-9 and MMP-14 levels would be related with TNM stage,extrapulmonary metastasis and poor prognosis,especially serum miR-9 and MMP-14 would be a certain predictive value on poor prognosis of advanced NSCLC patients.
