摘要
目的:对比改良RUSS评分、S.T.O.N.E.评分、CROES评分对肾结石术后结石清除的价值。方法:回顾性分析2018年1月—2019年12月期间我院手术治疗的189例肾结石患者的临床资料,术前均采用改良RUSS评分、S.T.O.N.E.评分、CROES评分评价。统计结石清除率;对比结石清除和残留患者术前改良RUSS评分、S.T.O.N.E.评分、CROES评分;绘制受试者工作特征曲线(ROC)分析其预测结石清除的效能,对比三者预测结石清除的曲线下面积(AUC)。结果:结石清除率为85.19%,实施输尿管软镜钬激光碎石术和经皮肾镜取石术者结石清除率分别为87.63%、82.61%;结石清除患者术前改良RUSS评分、S.T.O.N.E.评分、CROES评分均低于结石残留患者(P<0.05),且不同术式结石清除与结石残留患者对比结果一致;改良RUSS评分、S.T.O.N.E.评分预测结石清除的AUC均高于CROES评分(P<0.05),且在实施输尿管软镜钬激光碎石术和经皮肾镜取石术的患者中结果一致。结论:改良RUSS评分、S.T.O.N.E.评分、CROES评分均可预测肾结石术后结石清除,但改良RUSS评分、S.T.O.N.E.评分的预测效能优于CROES评分。
Objective:To compare the values among modified RUSS score,S.T.O.N.E.score and CROES score in stone clearance after operation for renal calculi.Methods:The clinical data of 189 cases of renal calculi treated by operation were retrospectively analyzed.All patients were evaluated with modified RUSS score,S.T.O.N.E.score and CROES score before operation.The stone clearance rate was counted.The modified RUSS score,S.T.O.N.E.score and CROES score were compared between the patients with stone removal and the patients with residual stone.Receiver operating characteristic curve(ROC)was drawn to analyze the effectiveness of ROC in predicting stone clearance,and the area under the curve(AUC)of the three methods was compared.Results:The stone clearance rate was 85.19%,and the stone clearance rates of flexible ureteroscopic holmium laser lithotripsy and percutaneous nephrolithotomy were 87.63%and 82.61%,respectively.The modified RUSS score,S.T.O.N.E.score and CROES score of patients with stone removal were lower than those of patients with residual stone(P<0.05),and the results of different stone removal methods were consistent with those of patients with residual stone.The AUC of modified RUSS score,S.T.O.N.E.score in predicting stone clearance were higher than that of CROES score(P<0.05),and the results were consistent in patients undergoing flexible ureteroscopic holmium laser lithotripsy and percutaneous nephrolithotomy.Conclusion:The modified RUSS score,S.T.O.N.E.score and CROES score can predict the stone clearance after operation for renal calculi,but the modified RUSS score and S.T.O.N.E.score are better than CROES score.
