双能量CT虚拟单能量成像技术在头颈CTA图像优化中的应用 被引量:10

Application of Dual-Energy CT Virtual Monoenergetic Imaging Algorithms in Head and Neck Angiography Image Optimization

摘要 目的探讨双能量CT虚拟单能量成像(VMI)技术对头颈CTA图像质量及检测狭窄的诊断效能的影响。方法对40例患者行双能量CT头颈部CTA检查,经后处理获得线性120 kVp、非线性融合及虚拟单能量3组图像。测量颈总动脉、颈内动脉、大脑中动脉及基底动脉的CT值,计算对比噪声比(CNR);对4组动脉的整体图像质量进行主观评分;对3组图像颈动脉狭窄程度进行检测和分级,并对结果进行统计学分析。结果 3组图像上4组动脉的CT值及CNR值差异具有统计学意义(P均<0.001);60 keV VMI与非线性融合组颈总动脉及颈内动脉CNR值差异无统计学意义(P均>0.05)。非线性融合组大脑中动脉及基底动脉CNR值大于60 keV VMI组(P均<0.05)。60 keV VMI及非线性融合图像的主观评分均优于线性120 kVp组(P均<0.001)。对于颈动脉狭窄的检测和分级,3组间无显著差异。结论 60 keV VMI为评价颈动脉和颅内动脉提供了较好的主观和客观图像质量,但其对颈动脉狭窄的诊断效能未见明显提高。此外,非线性融合技术对颅内动脉的显示效果优于60 keV VMI技术。 Objective To explore the effect of dual energy CT virtual monoenergetic reconstruction on head and neck CTA image quality and the diagnostic efficiency of stenosis detection. Methods Forty patients underwent dual-energy CT head and neck CTA examination. Afterpost-processing, the images of linear blended 120 kVp, non-linear blending and virtual monoenergetic reconstruction were obtained. The CT values of the common carotid artery, internal carotid artery, middle cerebral artery and basilar artery were measured, then CNR was calculated. The statistical analysis was performed. The overall image quality of the four groups of arteries was scored subjectively. The degree of carotid artery stenosis was detected and graded on the three groups of images. Results The CT values and CNR of the four groupsof arteries on the three groups of images were statistically significant(all P<0.001).In the 60 keV VMI and non-linear blending group, there was no statistically significant difference in the CNR of the common carotid artery and the internal carotid artery(all P>0.05).The CNR of middle cerebral artery and basilar artery in the non-linear blending group was higher than 60 keV VMI group( all P < 0. 05). The subjective scores of 60 ke V VMI and non-linear blending group were better than those of linearblended120 k Vp group( all P < 0. 001). For the detection and grading of carotid artery stenosis,there was no significant difference among three groups. Conclusion 60 ke V VMI provided better subjective and objective image quality for the evaluation of carotid and intracranial arteries,but its diagnostic efficacy for carotid stenosis was not significantly improved. Inaddition,the non-linear fusion technique is superior to the 60 ke V VMI technique in the display of intracranial arteries.
