摘要
目的探讨肾脏磁共振(MRI)小视野(rFOV)高分辨扩散加权成像(DWI)在肾脏肿瘤诊断中的应用价值。方法选取80例疑似肾脏肿瘤病变患者作为研究对象,对入组患者均行肾脏单次激发平面回波成像(SS-EPI)DWI和rFOV DWI检查,并行病理组织学检查诊断肿瘤的良恶性、病理类型和病理分级,比较80例患者SS-EPI DWI和rFOV DWI图像的质量分级情况,以及对不同肾脏肿瘤病灶的弥散系统(ADC)值,并作ROC曲线分析ADC值对不同肾脏肿瘤的诊断效能。结果经病理诊断,80例肾脏肿瘤病变患者中65例为肾恶性肿瘤,15例为肾脏良性肿瘤。DWI检查结果显示,rFOV DWI的3级图像患者例数低于SS-EPI DWI(P<0.05),4级+5级的优良质量图像患者例数高于SS-EPI DWI(P<0.05);rFOV DWI对良、恶性肾脏肿瘤和不同病理级别肾透明细胞癌的ADC值均明显高于SS-EPI DWI(P<0.05)。ROC曲线分析结果显示,rFOV DWI的ADC值诊断肾透明细胞癌、高级别肾透明细胞癌的ROC曲线下面积均高于SS-EPI DWI诊断(P<0.05)。结论相比常规DWI序列检查,高分辨rFOV DWI诊断肾脏肿瘤可明显提高DWI图像质量,其ADC值对肾透明细胞癌及其病理分级具有较高的诊断效能。
Objective To explore the application value of high-resolution reduced field of vision(rFOV)diffusion weighted imaging(DWI)of renal magnetic resonance imaging(MRI)in the diagnosis of renal tumors.Methods A total of 80 patients with suspected renal tumor lesions were enrolled as the research objects.All underwent renal single-shot echoplanar-imaging(SS-EPI)DWI and rFOV DWI.And histopathological examination was conducted to diagnose benign and malignant tumors,pathological types and pathological grades.The images quality grades of SS-EPI DWI and rFOV DWI among the 80 patients,as well as apparent diffusion coefficient(ADC)values in patients with different renal tumor lesions were compared.ROC curves were drew to analyze the diagnostic efficiency of ADC values for different renal tumors.Results The pathological diagnosis showed that of the 80 patients with renal tumor lesions,there were 65 cases with malignant tumors and 15 cases with benign tumors.The results of DWI examination showed that the number of patients with rFOV DWI images at grade 3 was less than that with SS-EPI DWI(P<0.05),while number of patients with excellent quality images at grade 4+5 was more than that with SS-EPI DWI(P<0.05).ADC values of rFOV DWI for diagnosis of benign and malignant tumors renal tumors,and renal clear cell carcinoma of different pathological grades were significantly higher than those of SS-EPI DWI(P<0.05).The results of ROC curve analysis showed that the area under the ROC curves(AUC)of rFOV DWI for diagnosis of renal clear cell carcinoma and high-grade renal clear cell carcinoma was higher than that of SS-EPI DWI(P<0.05).Conclusion Compared with routine DWI sequence examination,high-resolution rFOV DWI can significantly improve the quality of DWI images in the diagnosis of renal tumors,whose ADC values are of higher diagnostic efficiency for renal clear cell carcinoma and its pathological grading.
