摘要
目的探讨疾病诊断相关分组(DRGs)评价工具在医院质量管理与临床科室绩效评价中的应用。方法利用浙江省DRGs分组器分析2018年与2019年医院整体与临床科室DRGs指标,包括病例组合指数、相对权重以及三四级手术比例情况。结果2019年的各项医疗质量与绩效评价数据相比2018年均有所提高,其中CMI值增幅为1.24%,平均住院日及费用指标均有下降,同时三四级手术比例上升。临床科室方面,总体CMI值、RW≥2占比呈上升趋势,而平均住院日及三四级手术略有下降,其中外科A的CMI值为3.36,三四级手术比例为93.93%,为外科最高科室;内科F的CMI值为2.43,RW≥2占比为41.18%,为内科最高科室。结论医院综合服务能力与质量安全不断提升,同时能兼顾效率与费用,但应注重调整收治病种结构,注重学科均衡发展,提升综合能力。
Objective To explore the application of evaluation tools related to disease diagnosis groups(DRGs)in hospital quality management and clinical department performance evaluation.Methods Using Zhejiang DRGs grouper to analyze the DRGs indicators of the hospital as a whole and clinical departments in 2018 and 2019,Including the case mix index,relative weights,and the proportions of third or fourth grades of surgery.Results The various medical quality and performance evaluation data in 2019 had improved compared with 2018,and the CMI value increased by 1.24%.The average length of stay and cost indicators had decreased,while the proportion of third or fourth grades operations has increased.In terms of clinical departments,the overall CMI value and the proportion of RW≥2 were on the rise,while the average hospital stay and third or fourth grades operations had declined slightly.Among them,the CMI value of surgery A was 3.36,and the proportion of third and fourth grades operations was 93.93%,which was the highest department of surgery;The CMI value of internal medicine F was 2.43,and the proportion of RW≥2 was 41.18%,which was the highest department of internal medicine.Conclusion The hospital's comprehensive service capabilities and quality and safety have been continuously improved,while taking into account both efficiency and cost.However,attention should be paid to adjusting the structure of disease types,focusing on the balanced development of disciplines,and enhancing comprehensive capabilities.
