摘要
目的探讨零切迹椎间融合器治疗单节段颈椎病的疗效。方法采用零切迹椎间融合器治疗48例单节段颈椎病患者。记录手术时间、术中出血量、并发症发生情况、骨融合率、吞咽困难情况。比较手术前后JOA评分、颈椎功能障碍指数(NDI)、Odom评级、颈椎Cobb角、手术节段椎间隙高度百分比。结果患者均获得随访,时间1.5~4.0年。手术时间80~120 min,术中出血量20~120 ml。1例切口脂肪液化,加强换药后切口愈合良好;2例轻度吞咽困难,给予营养神经、流质饮食等对症处理2周后症状完全消失。均未发生零切迹椎间融合器螺钉断裂、沉降、松动及移位等并发症。JOA评分、NDI、颈椎Cobb角、椎间隙高度百分比术后各时间点均明显优于术前(P <0.05);末次随访时Odom评级优良率为95.8%(46/48)。骨融合率:术后6个月时为95.8%(46/48),末次随访时为100.0%(48/48)。结论零切迹椎间融合器治疗单节段颈椎病具有手术操作简单、手术时间短、术中显露范围小、术中出血量少、并发症发生率低等优点,术后颈椎生理曲度、椎间高度恢复满意。
Objective To investigate the efficacy of zero-profile interbody fusion cage in the treatment of single-level cervical spondylosis. Methods The 48 patients with single-level cervical spondylosis were treated with zero-profile interbody fusion cage. The operation time,intraoperative blood loss,complications and bone fusion rate,dysphagia situation were recorded. JOA score,cervical neck disability index( NDI),Odom grade,cervical Cobb angle and percentage of intervertebral space height in surgical segment were compared before and after operation. Results All patients were followed up for 1. 5 ~ 4. 0 years. The operation time was 80 ~ 120 min,and the intraoperative blood loss was20 ~ 120 ml. One case had incision fat liquefaction,and the wound healing was good after strengthening dressing change;2 cases were with mild dysphagia,who were given nerve nutrition,liquid diet and other symptomatic treatment,and the symptoms completely disappeared after 2 weeks. There were no complications such as screw breakage,subsidence,loosening and displacement of the zero-profile interbody fusion cage. The JOA score,NDI,cervical Cobb angle,the percentage of height of intervertebral space were significantly better than those before operation( P < 0. 05);at the last follow-up,the excellent-good rate of Odom grade was 95. 8%( 46/48). Bone fusion rate was 95. 8%( 46/48) at 6 months postoperatively,and 100. 0%( 48/48) at the last follow-up. Conclusions In the treatment of single-level cervical spondylosis,zero-profile anterior cervical fusion cage has the advantages of simple operation,short operation time,small intraoperative exposure range,less intraoperative blood loss,low incidence rate of complications,and satisfactory recovery of cervical curvature and intervertebral height.
