摘要
目的回顾性总结踝关节距骨骨软骨损伤的磁共振影像学表现,分析其特点并进行性分型,评估磁共振影像学检查在距骨骨软骨损伤中的实际临床诊断价值。方法选取郑州市骨科医院2018—2019年68例踝关节距骨骨软骨损伤的患者作为本次研究对象。所有观察对象均进行了磁共振检查,应用PACS系统测量软件进行分析,并进行治疗后随访。结果采用磁共振Hepple’s分型标准得出结果:Ⅰ型2例,Ⅱ型26例,Ⅲ型16例,Ⅳ型6例,Ⅴ型18例。根据损伤位置分布:磁共振冠状位扫面显示病变位于内侧38例(56%),外侧24例(35%),内外侧具有6例(9%);磁共振矢状位显示,10例病灶位于前部(15%),37例位于中部(54%),14例位于后部(21%),7例混合型(10%)。距骨骨软骨损伤主要集中于内侧中部上方及外侧中部,以内侧更为明显。结论磁共振检查能明确显示骨软骨损伤的部位、程度、范围,对于距骨骨软骨损伤诊断价值显著,有助于临床明确诊断、选择治疗方案及手术入路,值得广泛运用于临床诊断。
Objective To retrospectively summarize the magnetic resonance imaging manifestations of ankle joint talar osteochondral injury, analyze its characteristics and perform classification, and evaluate the actual clinical diagnostic value of magnetic resonance imaging in the talar osteochondral injury. Methods Totally 68 patients with ankle joint talar osteochondral injury from 2018 to 2019 in our hospital were selected as the research objects. All observation subjects underwent magnetic resonance examination, applied PACS system measurement software for analysis, and followed up after treatment. Results We used MRI Hepple’s classification criteria to obtain results: 2 cases of type I, 26 cases of type II, 16 cases of type III, 6 cases of type IV, and 18 cases of type V. According to the distribution of injury location: MRI coronal scan showed that the lesions were located on the medial side in 38 cases(56%), laterally in 24 cases(35%), and on the medial and lateral side in 6 cases(9%);MRI showed in sagittal view, 10 cases of lesion Located in the front(15%), 37 cases were in the middle(54%), 14 cases were in the back(21%), and 7 cases were mixed(10%). Talar osteochondral damage is mainly concentrated on the upper medial middle part and the lateral middle part, and is more obvious on the medial side. Conclusion Magnetic resonance imaging can clearly show the location, extent, and scope of osteochondral injury. It is of great value in the diagnosis of talar osteochondral injury. It is helpful for clinical diagnosis, selection of treatment options and surgical approaches, which are worthy of being widely used in clinical diagnosis.
