摘要
目的探讨内镜黏膜切除术在治疗结直肠侧向发育型肿瘤中的应用疗效。方法研究样本选自2017年12月~2020年9月某院救治的60例结直肠侧向发育型肿瘤患者,根据患者的实际病况,将其随机分为两组,其中对照组给予内镜黏膜下剥离术治疗,观察组给予内镜黏膜切除术,各30例。评估两组的病理特征、围术期指标、术后出血率、复发率、整块切除率。结果两组的病变大小、病变位置、分型、术中穿孔率、术后出血率相比差异无统计学意义(P>0.05);观察组的内镜操作时间显著低于对照组(P<0.05);观察组的复发率显著高于对照组(P<0.05);在直径为1~2 cm的病变中,观察组的整块切除率为82.14%,对照组的整块切除率可达100.00%,两组差异无统计学意义(P>0.05);在直径超出2 cm的病变中,观察组中的2例均没有实现整块切除,对照组的整块切除率为92.31%,显著高于观察组(P<0.05)。结论内镜黏膜切除术与内镜黏膜下剥离术在治疗结直肠侧向发育型肿瘤中各有其自身优势,在施行结直肠侧向发育型肿瘤内镜治疗时,必须依据具体临床状况选用针对性的治疗手段,其中内镜黏膜切除术的操作时间较短,但对于直径>2 cm的病灶更适合利用内镜黏膜下剥离术治疗。
Objective To investigate the efficacy of endoscopic mucosal resection in the treatment of lateral colorectal tumors.Methods 60 patients with lateral colorectal tumor treated in our hospital were selected and treated from December 2017 to September 2020.According to the actual condition of patients,they were randomly divided into two groups,the control group was given endoscopic submucosal dissection,the observation group was given endoscopic mucosal resection,30 cases in each group.The pathological characteristics,perioperative indexes,postoperative bleeding rate,recurrence rate and total resection rate of the two groups were evaluated.Results There was no significant difference in the size,location,type,perforation rate and bleeding rate between the two groups(P>0.05);the endoscopic operation time of the observation group was significantly lower than that of the control group(P<0.05);the recurrence rate of the observation group was significantly higher than that of the control group(P<0.05);in the lesions with a diameter of 1~2 cm,the total resection rate of the observation group was 82.14%,and that of the control group was 100.00%,and there was no significant difference between the two groups(P>0.05);in the lesions with diameter more than 2 cm,2 cases in the observation group did not achieve the whole resection,and the total resection rate of the control group was 92.31%,which was significantly higher than that of the observation group(P<0.05).Conclusion Endoscopic mucosal resection and endoscopic submucosal dissection have their own advantages in the treatment of colorectal laterally developed tumors.In the endoscopic treatment of colorectal laterally developed tumors,targeted treatment must be selected according to the specific clinical conditions.The operation time of endoscopic mucosal resection is shorter,but endoscopic submucosal dissection is more suitable for lesions with diameter greater than 2 cm.
