促炎性饮食与消化系统肿瘤患病风险的meta分析 被引量:1

摘要 目的评价促炎性饮食与消化系统肿瘤患病风险的关系,为健康饮食提供科学指导。方法截至2020年12月,系统检索英文数据库PubMed、WebofScience、Embase、CochraneLibrary和中文数据库万方数据期刊论文资源、中国学术期刊网络出版总库、维普中文科技期刊全文数据库筛选出饮食炎症效应与消化系统肿瘤患病风险相关的文献。纳入文献均采用膳食炎症指数(DII)评价饮食的潜在炎症效应。采用RevMan 5.2软件进行meta分析,合并纳入研究的OR及95%CI,同时根据研究类型、肿瘤类型和地域进行亚组分析。结果共纳入文献20篇,参与者974024名。meta分析结果显示:最高促炎性饮食的个体与最高抗炎性饮食的个体比较,消化系统肿瘤患病风险增加(OR=1.63,95%CI=1.45~1.83)。按肿瘤类型进行亚组分析结果表明,促炎性饮食模式下,食管癌患病风险最高(OR=2.86,95%CI=1.60~5.11);胃癌患病风险显著增加(OR=2.00,95%CI=1.34~2.99);结肠癌患病风险增加(OR=1.42,95%CI=1.21~1.66);直肠癌患病风险亦增加(OR=1.33,95%CI=1.11~1.61)。因肝癌和胰腺癌各纳入1篇文献,未进行分析。按地域进行亚组分析显示:促炎性饮食模式下消化系统肿瘤患病风险在亚洲OR值为1.82(95%CI=1.67~1.99)、欧洲为1.81(95%CI=1.56~2.09)、美洲为1.27(95%CI=1.21~1.34)。结论促炎性饮食可能会增加消化系统肿瘤患病风险,尤其是食管癌。亚洲地区患病风险可能高于美洲和欧洲。 Objective This meta⁃analysis aimed to evaluate the relationship between pro⁃inflammatory diet and the risk of gastrointestinal cancer,so as to provide scientific guidance for a healthy diet.Methods The PubMed,Embase,Web of Science,Cochrane Library,CNKI,Wanfang Data and VIP Database were retrieved by computer.According to the inclusion criteria,the related studies before December 2020 were collected and evaluated.Dietary inflammation index(DII)was used to evaluate the inflammatory effect of diet.This meta⁃analysis was carried out by RevMan 5.2 software.The odds ratio(OR)value and 95%confidence interval(CI)of the included studies were pooled.At the same time,the subgroup analysis was conducted according to the cancer type and continent.Results A total of twenty literatures were included,which enrolled 974024 cases.Individuals in the highest pro⁃inflammatory diet category were increased with the incidence of digestive system cancer compared to those in the highest anti⁃inflammatory diet category(OR=1.63,95%CI=1.45-1.83).Subgroup analysis by cancer type showed that individuals with the highest pro⁃inflammatory diet category were increased with esophageal cancer(OR=2.86,95%CI=1.60-5.11),stomach cancer(OR=2.00,95%CI=1.34-2.99),colon cancer(OR=1.42,95%CI=1.21-1.66),rectal cancer(OR=1.33,95%CI=1.11-1.61).Because there was only one study for liver cancer and pancreatic cancer,no subgroup analysis was performed.Subgroup analysis by continent showed that gastrointestinal cancer risk from a pro⁃inflammatory diet is:Asia(OR=1.82,95%CI=1.67-1.99),Europe(OR=1.81,95%CI=1.56-2.09),America(OR=1.27,95%CI=1.21-1.34).Conclusion Dietary patterns with pro⁃inflammatory diet may increase the risk of digestive system cancer,especially esophageal cancer.The correlation between pro⁃inflammatory diet and digestive system cancer risk in Asia may be higher than that in Europe and North America,but this conclusion needs to be confirmed by more studies.Healthy diet can be used as one of the important measures to prevent digesti
