摘要
目的探讨急腹症患者应用全面腹部超声检查的临床价值。方法采用回顾性研究方法,选取2018年3月—2020年3月医院急诊科收治的88例急腹症患者作为研究对象,对其均行全面性及选择性腹部超声检查,随后患者均行手术病理检查,以手术病理检查结果作为"金标准"。对患者行全面性及选择性腹部超声检查与病理结果的诊断符合率、超声检查时间、临床诊断时间、检查费用比较。结果全面腹部超声检查结果和"金标准"符合的患者共计86例,诊断符合率为97.73%(86/88);选择性腹部超声检查和"金标准"符合的患者共计72例,诊断符合率为81.82%(72/88);全面腹部超声检查的诊断符合率显著高于选择性腹部超声检查且差异有统计学意义(χ^(2)=12.129,P=0.000)。全面腹部超声检查的检查时间比选择性腹部超声检查长,临床诊断时间比选择性腹部超声检查短,检查费用比选择性腹部超声检查高且差异有统计学意义(P<0.05)。结论与选择性腹部超声检查相比,全面腹部超声检查检查时间略长,检查费用略高,但是其可以显著提高急腹症的诊断符合率,缩短临床诊断时间,并为临床诊疗提供更加有效的依据及指导意见。
Objective To explore the clinical value of comprehensive abdominal ultrasound examination in patients with acute abdomen.Methods A retrospective study was conducted on 88 patients with acute abdomen treated in emergency department from March 2018 to March 2020,comprehensive and selective abdominal ultrasound examination was performed,then all the patients underwent surgical and pathological examination,and the results of surgical and pathological examination were taken as the"gold standard".The diagnostic coincidence rate,ultrasound time,clinical diagnosis time and examination cost of comprehensive and selective abdominal ultrasound examination and pathological results were compared.Results A total of 86 patients met the"gold standard"results of the comprehensive abdominal ultrasound examination,and the diagnostic compliance rate was 97.73%(86/88);a total of 72 patients met the gold standard for selective abdominal ultrasound examination,and the diagnostic compliance rate was 81.82%(72/88).The diagnostic coincidence rate of comprehensive abdominal ultrasonography was significantly higher than that of selective abdominal ultrasonography and the difference was statistically significant(χ^(2)=12.129,P=0.000).The examination time of comprehensive abdominal ultrasonography was longer than that of selective abdominal ultrasonography,and the clinical diagnosis time was shorter than that of selective abdominal ultrasonography.The inspection cost was higher than that of selective abdominal ultrasonography and the difference was statistically significant(P<0.05).Conclusion Compared with selective abdominal ultrasound,comprehensive abdominal ultrasound examination has a slightly longer time and a slightly higher cost,but it can significantly improve the diagnostic coincidence rate of acute abdomen and shorten the clinical diagnosis time,and to provide more effective basis and guidance for clinical diagnosis and treatment.
