摘要
目的探讨睑板腺功能障碍(MGD)型干眼与螨虫感染的相关性及药物治疗的效果评估。方法选取2019年6月至2020年5月在山西白求恩眼科门诊确诊为MGD型干眼患者90例(90眼)为试验组,其中男性42例,女性48例,年龄18~60岁,平均(42±7)岁。按照干眼的严重程度分3组,选取无MGD健康人30名(30眼)作为对照组,检测2组患者的螨虫的感染率。试验组中选取螨虫感染的MGD型干眼患者55例,其中男性24例,女性31例,年龄31~58岁,平均(40±5)岁,随机分成2组,组A予局部睑板腺按摩+妥布霉素治疗,辅助每日热敷30 min。组B在此治疗基础上加用茶树油清洁湿巾(医用冷敷护眼敷料)。治疗4周后检测螨虫清除率及干眼好转率。结果 MGD型干眼3组患者与对照组比较,螨虫感染率差异有统计学意义(F=5.43,P=0.017)。对螨虫感染的MGD型干眼药物治疗的结果比较,A、B 2组患者治疗前后裂隙灯下观察临床体征评分比较,2组间差异有统计学意义(t=-2.417,P=0.008)。干眼好转率差异有统计学意义(χ^(2)=3.14,P=0.026);螨虫清除率的比较,2组差异有统计学意义(χ^(2)=5.61,P=0.004)。结论在MGD型干眼患者中,螨虫感染率较健康人群高。并随着干眼程度的加重,螨虫感染率也逐渐增高。在综合药物治疗时,加用茶树油清洁湿巾(医用冷敷护眼敷料)杀螨效果更好,对于改善干眼的症状及眼部体征的评分均优于对照组。
Objective To investigate the correlation between dry eyes with meibomian gland dysfunction(MGD)and mite infection,and to evaluate the effect of drug treatment.Methods A prospective cohort study was conducted.Ninety patients(90 eyes)diagnosed with MGD-type dry eyes at Department of Ophthalmology,Shanxi Bethune Hospital between June 2019 and May 2020 were recruited in the study group,including 42 males and 48 females,aged 18~60 years old[mean age(42±7)years old].According to the severity of dry eyes,all patients were divided into 3 subgroups.Thirty normal subjects(30 eyes)without MGD were included in the control group.The infection rate of mites in the two groups was measured.In the study group,55 patients with MGD-type dry eyes and mite infection were selected,including 24 males and 31 females,aged 31~58 years old[mean age(40±5)years old],and were randomly divided into two subgroups.Group A was treated with local meibomian gland massage+Tobramycin,assisted by daily hot compress for 30 minutes.In addition,group B was treated with tea tree oil cleaning wet wipes(medical cold compress dressing for eye protection).At 4 weeks after the treatment,the mite removal rate and dry eyes improvement rate were determined.Results There was statistically significant difference in the mite infection rate between the 3 groups of MGD-type dry eyes and the control group(F=5.43,P=0.017).The results of drug treatment for patients with MGD-type dry eyes and mite infection were compared.There was statistically significant difference in the scores of clinical signs under slit-lamp examination at baseline and after the treatment between groups A and B(t=-2.417,P=0.008).There were statistically significant difference in the improvement rate of dry eyes(χ^(2)=3.14,P=0.026)and mite removal rate(χ^(2)=5.61,P=0.004)between the two groups.Conclusion In patients with MGD-type dry eyes,the rate of mite infection is higher than that in normal subjects.The rate of mite infection increases along with the aggravating dry eyes.In the comprehensive
