摘要
目的:探讨原发性骨质疏松症(Primary-osteoporosis,POP)是否与认知功能减退存在相关性。方法:2017年9月-2018年4月在北京市17个社区招募符合要求的60岁以上居民578例,利用双能X线骨密度测量仪对受试者的腰椎、左髋和右髋骨密度(Bone Mineral Density,BMD)进行测定,使用简明精神状态检查量表(Mini-mental State Examination,MMSE)和画钟试验(Clock Drawing Task,CDT)对受试者进行认知功能评估及血清骨代谢生化指标检查。结果:578例研究对象中非骨质疏松症361例,骨质疏松症患者217例。非骨质疏松组MMSE评分显著高于骨质疏松组(P=0.001),MMSE评分与骨量呈负相关(r=-0.142,P=0.001),二元Logistic回归校正性别、年龄、受教育年限和常见基础疾病后MMSE评分和骨量仍存在相关性(OR=0.925,P=0.0035)。两组CDT评分差异无统计学意义(P=0.076)。MMSE与Ⅰ型胶原羧基端肽β特殊序列(β-C-temlinal telopeptide of type Ⅰ collagen,β-CTX)呈负相关(r=-0.106,P=0.010),与甲状旁腺素(Parathyroid Hormone,PTH)、骨钙素(Osteocalcin,OC)、Ⅰ型原胶原蛋白氨基末端前肽(N-terminal Propeptide of type Ⅰ Procollagen Protein,PINP)无关,校正混杂因素后均无相关性。CDT与骨代谢指标均无关。结论:POP和认知功能减退存在正相关,但仍需进一步开展前瞻性研究。
Objective:To explore the correlation between primary osteoporosis and cognitive decline.Methods:578 residents were recruited to this study whose age over 60 years old and living in Beijing.The bone mineral density of the lumbar spine,left hip and right hip were measured by dual-energy X-ray bone mineral density.The cognitive function of the subjects was assessed by MMSE and CDT.Results:Among the 578 subjects,361 were non-osteoporosis patients and 217 were osteoporosis patients.MMSE score in the nonosteoporosis group was significantly higher than that in the osteoporosis group(P=0.001),and MMSE score was negatively correlated with bone mass(r=-0.142,P=0.001).There was still a correlation between MMSE score and bone mass after binary Logistic regression analysis adjusted for gender,age,years of education and general basic diseases(OR=0.925,P=0.0035).There was no significant difference in CDT scores between the two groups(P=0.076).MMSE was negatively correlated withβ-CTX(r=-0.106,P=0.010),without PTH,OC and PINP.There was no correlation between CDT and bone metabolism.Conclusion:There is a positive correlation between primary osteoporosis and cognitive decline,but further prospective studies are needed.
