摘要
目的:分析单纯孔源性视网膜脱离(RRD)患者行微创巩膜外加压术后复位和视力恢复的相关影响因素。方法:研究对象为笔者所在医院在2018年8月-2020年8月收治的150例(150眼)采用微创巩膜外加压术作为临床治疗方案的RRD患者,对其手术治疗后的视网膜复位成功率进行统计,同时观察其术后BCVA,分析其术后复位以及视力恢复的相关影响因素。结果:150例患者术后共计138眼(92.00%)复位成功,其中视网膜脱离范围、裂孔数及PVR分级是影响其术后复位成功与否的主要影响因素(P<0.05)。150例患者术后6个月BCVA≥0.4者共计98眼(65.33%),其中术前BCVA、病程、视网膜脱离范围及黄斑状态和患者手术治疗后视力恢复密切相关(P<0.05)。多因素Logistic回归分析结果表明影响患者手术治疗后视力恢复的独立危险因素为术前BCVA(P<0.05)。结论:在RRD患者的临床治疗中,微创巩膜外加压术具有良好的治疗效果,术前BCVA是影响其术后视力恢复的独立危险因素,因此对于本病需要做到早发现早治疗。
Objective:To analyze the related factors of reduction and visual recovery in patients with simple rhegmatogenous retinal detachment (RRD) after minimally invasive extrascleral compression.Method:The research objects were 150 RRD patients (150 eyes) treated with minimally invasive episcleral compression surgery in our hospital from August 2018 to August 2020.The success rate of retinal reattachment after surgical treatment was calculated.At the same time,the postoperative BCVA was observed,and the related factors affecting postoperative reduction and visual recovery were analyzed by multi-factor analysis and multi-factor logistic regression analysis.Result:A total of 138 eyes (92.00%) of 150 patients were successfully repositioned after operation.The range of retinal detachment,the number of tears,and the PVR grade were the main factors affecting the success of postoperative reposition (P<0.05).A total of 98 eyes (65.33%) of 150 patients were BCVA≥0.4 6 months after operation.Among them,preoperative BCVA,course of disease,range of retinal detachment,and macular state were risk factors affecting their postoperative vision recovery (P<0.05).Multivariate logistic regression analysis showed that the independent risk factor affecting postoperative vision recovery was preoperative BCVA (P<0.05).Conclusion:In the clinical treatment of RRD patients,minimally invasive extrascleral compression has a good therapeutic effect.Preoperative BCVA is an independent risk factor that affects the recovery of vision after surgery.Therefore,early detection and early treatment of this disease is necessary.
