摘要
目的:应用失效模式与效应分析法(FMEA),开展干预研究,降低重症医学科医院感染风险,保障医疗安全。方法:运用FMEA技术,建立风险评估小组,分析导致重症医学科医院感染高发的失效模式及失效原因,找出需要改进的失效模式,制定行之有效的干预措施,实行为期三个月的干预研究。比较干预组和对照组患者医院感染情况,评价干预效果。结果:干预组的医院感染率、感染例次率、呼吸机相关性肺炎发病率、中心静脉导管相关血流感染发病率均低于对照组;干预组医务人员手卫生依从率和正确率均有提高;ICU工作人员卫生手、物体表面、地面、器械的清洁消毒综合监测合格率呈上升趋势。结论:合理运用FMEA分析法,能降低ICU医院感染风险,提高ICU医院人员手卫生率和正确率,在提高医务人员卫生手、物体表面、地面、器械的清洁消毒效果方面起积极作用。
Objective:UsingFailure Mode and Effects Analysis(FMEA)method,to develop interventions studio,and to reduce the risk of nosocomial infection in ICU and ensure medical safety.Methods:Using FMEA technology,a risk assessment team was estab⁃lished to analyze the failure modes and reasons leading to the high incidence of nosocomial infection in the department of serious ill⁃ness medicine,to find out the failure modes that need to be improved,to develop effective intervention measures,and to carry out a three-month intervention study.The nosocomial infection of patients in the intervention group and the control group were compared to evaluate the intervention effect.Results:The nosocomial infection rate,and the nosocomial infection rate,the incidence of ventilators associated pneumonia,the incidence of central venous catheter-associated bloodstream infections in the intervention groupwas lower than that in the control group.The hand hygiene compliance rate and accuracy rate of medical staff in the intervention group were im⁃proved.The qualified rate of the comprehensive monitoring of cleaning and disinfection of ICU staff's sanitary hands,surface of objects,ground and instruments showed a rising trend.Conclusion:Reasonable use of FMEA analysis can reduce the risk of nosocomial infec⁃tion in ICU,improve the rate and accuracy of hand hygiene of ICU hospital staff,and play a positive role in improving the cleaning and disinfection effect of medical staff on hand hygiene,object surface,ground and instruments.
