摘要
目的:观察中药热敷联合手法松解治疗肩周炎的临床疗效。方法:回顾性分析58例肩周炎患者,根据治疗方法不同分为观察组和对照组各29例。对照组给予手法松解治疗,观察组在对照组基础上加用中药热敷治疗。比较2组临床疗效、治疗前后肩部疼痛程度、肩关节功能及肩部活动情况。结果:观察组总有效率为96.55%,明显高于对照组的68.97%,差异有统计学意义(P<0.05)。治疗前,2组视觉模拟评分法(VAS)评分比较,差异无统计学意义(P>0.05)。治疗后,2组VAS评分较治疗前下降,且观察组VAS评分明显低于对照组,差异有统计学意义(P<0.05)。治疗前,2组肩关节功能分级及加州大学洛杉矶分校(UCLA)肩关节功能评分比较,差异无统计学意义(P>0.05)。治疗后,2组肩关节功能分级及UCLA肩关节功能评分较治疗前改善,且观察组改善程度优于对照组,差异均有统计学意义(P<0.05)。治疗前,2组胳膊外旋角度、外展角度、上举角度比较,差异无统计学意义(P>0.05)。治疗后,2组胳膊外旋角度、外展角度、上举角度较治疗前升高,且观察组胳膊外旋角度、外展角度、上举角度高于对照组,差异均有统计学意义(P<0.05)。结论:中药热敷联合手法松解治疗肩周炎患者疗效显著,可以改善患者肩部疼痛程度和肩部活动情况,恢复肩关节功能。
Objective: To observe the curative effect of Chinese medicine hot compress combined with manipulation release for scapulohumeral periarthritis. Methods:A total of 58 cases of patients with scapulohumeral periarthritis were given retrospective analysis and divided into the observation group and the control group according to different treatment methods,29 cases in each group.The control group was treated with manipulation release,and the observation group was additionally treated with Chinese medicine hot compress based on the treatment of the control group. The degree of shoulder pain,shoulder joint function and shoulder activities before and after treatment as well as the clinical effect in the two groups were compared. Results:The total effective rate was 96.55% in the observation group,significantly higher than that of 68.97% in the control group, the difference being significant(P<0.05). Before treatment, there was no significant difference being found in the comparison of Visual Analogue Scale(VAS) scores between the two groups(P>0.05). After treatment, VAS scores in the two groups were decreased when compared with those before treatment,and the VAS score in the observation group was significantly lower than that in the control group, the difference being significant(P<0.05). Before treatment,there was no significant difference being found in the comparison of the shoulder function classification and the University of California, Los Angeles(UCLA) Shoulder Score between the two groups(P>0.05). After treatment, the shoulder function classification and the UCLA Shoulder Score in the two groups were improved when compared with those before treatment,and the improvement in the observation group was better than that in the control group, differences being significant(P<0.05). Before treatment,there was no significant difference being found in the comparison of angles of extorsion,abduction and uplift in the arms between the two groups(P>0.05).After treatment,the angles of extorsion,abduction and uplift in the arm
