摘要
目的:探讨骨科主导多学科协作模式下加速康复外科(ERAS)在老年髋部骨折患者围术期中的应用效果。方法:选取2017年1月1日~2020年12月31日收治的587例老年髋部骨折患者作为研究对象,随机分为观察组315例和对照组272例;观察组采取骨科主导多学科协作模式下ERAS围术期护理,对照组采取常规围术期护理;比较两组手术时长、术中失血量、输血率,住院时间、治疗费用,术后并发症发生率,护理满意度,术后3个月髋关节功能Harris评分。结果:观察组术中失血量少于对照组(P<0.01),输血率低于对照组(P<0.01),住院时间短于对照组(P<0.05),治疗费用低于对照组(P<0.05),并发症总发生率低于对照组(P<0.01),患者对护理工作的总体满意度高于对照组(P<0.01),术后3个月Harris评分高于对照组(P<0.01)。结论:将骨科主导多学科协作模式下ERAS应用于老年髋部骨折患者围术期护理中,可减少术中失血量,降低输血率,缩短住院时间,减少治疗费用,降低并发症发生率,改善髋关节功能。
Objective:To explore the application effect of accelerated rehabilitation surgery(ERAS)in orthopaedic dominant multidisciplinary collaboration mode in perioperative elderly hip fracture patients.Methods:During January 1,2017 to December 31,2020,587 cases of elderly patients with hip fracture were selected as the research objects,and were randomly divided into the observation group with 315 cases and the control group with 272 cases;The observation group adopted the perioperative nursing in the orthopaedic dominant ERAS multidisciplinary collaboration mode,the control group adopted the routine perioperative nursing care;Comparison was made between the two groups on operation time,intraoperative blood loss,blood transfusion rate,length of hospital stay,cost of treatment,the incidence of postoperative complications,nursing satisfaction,and Harris hip joint function scores 3 months after surgery.Results:The intraoperative blood loss was lower in the observation group than that in the control group(P<0.01),the blood transfusion rate was lower than that in the control group(P<0.01),the hospital stay time was shorter than that in the control group(P<0.05),the treatment cost was lower than that in the control group(P<0.05),the total incidence of complications was lower than that in the control group(P<0.01),the overall satisfaction of patients with nursing work was higher than that in the control group(P<0.01),the postoperative 3 months Harris scores were higher than those in the control group(P<0.01).Conclusion:The application the orthopedic dominant ERAS multidisciplinary collaboration mode in perioperative nursing of elderly hip fracture patients can decrease intraoperative blood loss,lower blood transfusion rate,shorten hospital stay time,reduce the cost of treatment,reduce the incidence of complications,and improve the function of hip joint.
