摘要
目的:调查分析长沙本地老年髋部骨折的流行病学特征,为老年髋部骨折的预防及治疗提供理论依据。方法:通过长沙市人力资源与社会保障局数据库检索2014年1月1日~2018年12月31日期间出院诊断为股骨转子间骨折、股骨粗隆间骨折和股骨颈骨折病例数据信息,共检索病例总数9706例,其中159例病例因中途转院造成信息重复登记,不予纳入最终数据分析,回顾性分析剩余9547例髋部骨折住院患者年龄、性别、手术病人住院天数、内置耗材费用及住院总费用。结果:2014年粗发病率为7.41/10万,2015年粗发病率为29.12/10万,2016年粗发病率为29.12/10万,2017年粗发病率为32.41/10万,2018年粗发病率为36.96/10万;以入院时年龄计算,60~64岁2303人(24.1%),65~69岁974人(10.2%),70~74岁1123人(11.8%),75~79岁1640人(17.2%),80岁以上共3507人(36.7%);3.手术治疗患者共6197人,平均住院天数为18.83±10.89天,平均内置耗材费用为17572.12±11816.79元,平均医疗总费用为40018.80±24240.69元。结论:髋部骨折发病率逐年上升;髋部骨折手术治疗患者数、平均内置耗材费用、手术患者住院总费用总体上均呈逐年下降趋势。
Objective To investigate the epidemiological characteristics of local elderly hip fractures in Changsha,and to provide theoretical basis for the prevention and treatment of elderly hip fracture.Methods Searched by Changsha Human Resources and Social Security Bureau database,the data of patients with femoral intertrochanteric fractures or femoral neck fractures who were discharged from January 1,2014 to December 31,2018 were retrieved.A total of 9706 cases were retrieved,of which 159 cases were repeatedly registered due to midway transfers,and final data analysis was not included.The remaining 9547 elderly hip fracture patients were retrospectively analyzed.All data including age,sex,length of hospital stay of surgical patients,Built-in consumables costs and total hospitalization expenses.Results 1.The crude incidence rate was 7.41/100000 in 2014,29.12/100,000 in 2015,29.12/100,000 in 2016,32.41/100,000 in 2017,and 36.96/100,000 in 2018;2.Based on the age at admission,2303(24.1%)were 60~64 years old,974(10.2%)were 65~69 years old,1123(11.8%)were 70~74 years old,and 1640(17.2%)were 75~79 years old.A total of 3507 people(36.7%)over the age of 80;3.A total of 6197 patients were treated with surgery.The average hospital stay was 18.83±10.89 days,the average cost of built-in consumables was 17572.12±11816.79,the average total medical cost was 40018.80±24240.69.Conclusion The incidence of hip fractures is increasing year by year;The number of patients with hip fracture surgery,the average cost of built-in consumables,and the total cost of hospitalization for surgery patients all showed a downward trend year by year.
