摘要
目的探讨改良的佩顿四步法在中医医院住院医师规范化培训中的洗胃术培训的可行性及初步效果。方法选择于2018年7月至2018年12月在广东省中医院急诊科轮科的规培生96人,将改良的佩顿四步法应用于洗胃术的技能培训课程中。通过描述性研究,调查学员自我等级评定、教学效果评价以及专家进行督导评级。结果学员自我等级评定显示,学员在培训前对洗胃相关的解剖结构知识、用物、步骤以及自信心均不理想,与培训后的自我效能评估对比,均存在统计学差异(均P<0.001)。认为通过培训能学到很多知识,反复观察、独立操作、相互点评和指导有利于掌握洗胃、认为通过培训可以在实际临床中胜任此操作的学员分别占95.2%、92.7%、97.9%、99%和90.6%。89.6%的学员在学习中能保持精神集中。认为培训的重复观察太少占40.7%,太多的占57.3%,有72名学员认为操作机会太少。专家督导的视觉模拟量表评价,分别为学员表现(87.92±6.53)分,教师表现(92.86±4.33)分,培训流程(87.49±5.78)分。结论改良的佩顿四步法应用于洗胃术的培训,能让学员能较好地接受,且教师易于实施,能减少一定的教学支出,缓解教学需求与资源之间的矛盾。
Objective:To develop and evaluate a modified Peyton′s Approach for small group teaching,and to explore the feasibility and preliminary effects of gastric lavage training in Residents Standardized Training in Chi⁃nese medicine hospitals.Methods:A total of 96 resident doctors receiving standardized training from July to De⁃cember 2018 in the emergency department of Guangdong Provincial Hospital of Traditional Chinese Medicine were recruited.A modified Peyton′s Approach was applied the in the skill training course of gastric lavage.Through de⁃scriptive research,investigating trainees′self-rating,teaching effect evaluation,and expert supervision and rating.Results:The self-rating of the trainees showed that the trainees′knowledge of anatomical structures,materials,procedures and self-confidence related to gastric lavage before the training were not ideal,and there were statisti⁃cal differences compared with the self-rating after the training(all P<0.001).The percentages of trainees who“have learned a lot during the training session”,“repeated observation of the procedure was helpful”,“indepen⁃dent performance of the procedure was helpful”,“commenting on and instructing the procedure was helpful”,“feel well prepared for clinical independently”were 95.2%,92.7%,97.9%,99%,and 90.6%;89.6%of the trainees con⁃tinuously alert during the training.However,40.7%of trainee felt that there were too few repeated observations of the procedure,and 57.3%of them felt too many.Furthermore,72 trainees thought there was few chances for inde⁃pendent performances.The evaluation of the visual analog scale supervised by independent experts was(87.92±6.53)points for trainees,(92.86±4.333)points for teachers,and(87.49±5.783)points for training process.Con⁃clusion:A modified Peyton′s Approach to instruct in the training of gastric lavage has revealed to be practicable,well accepted by trainees,and easy for teachers to realize.It can reduce a certain amount of teaching expenditure and ease the con
