摘要
目的探讨重症护理亚专科建设的模式及效果。方法根据重症护理专科特点,组建呼吸道管理、血液净化、重症营养、静脉治疗、血流动力学监测、康复治疗、造口伤口护理等7个护理亚专科学组,经过2年规范化培训的护士作为亚专科管理小组成员,制订亚专科组工作目标、工作流程、培训方案、质量控制。结果重症医学科质量指标呼吸机相关性肺炎发病率、中心静脉置管相关血流感染发病率、留置导尿管相关泌尿系统感染发病率均明显下降(P<0.05);护士专科理论水平、专科技术能力、核心能力均有显著提高(P<0.05)。结论重症医学科实施亚专科护理管理模式,可显著提高危重症护理质量,提高护士专科理论及技术水平,提升护士核心能力。
Objective To explore the construction model and effect of intensive care sub-specialty.Methods According to the characteristics of intensive care unit,to component 7 nursing sub-specialty groups including respiratory tract management,blood purification,critical nutrition,venous therapy,hemodynamic monitoring,rehabilitation,colostomy care.Nurses who have been trained for 2 years were the members in each sub-specialty team and they set the objectives,working process,training programs and the quality control of sub-specialty.Results The quality index of critical care unit decreased significantly(P<0.05),which included ventilator-associated pneumonia(VAP)rates,incidence of catheter-related blood stream infection(CRBSI)and catheter-related urinary tract infection(CAUTI).Nurses'theoretical level,technical ability and core competence significantly improved(P<0.05).Conclusion The implementation of sub-specialty nursing management model in ICU can significantly improve the quality of critical care,improve nurses'theoretical and technical level,and enhance their core competence.
