呼出气一氧化氮联合肺功能检测在儿童胸闷变异性哮喘诊断中的价值 被引量:18

Diagnostic value of fractional exhaled nitric oxide(FeNO)combined with pulmonary function parameters in children with chest tightness variant asthma

摘要 目的探讨呼出气一氧化氮(fractional exhaled nitric oxide,FeNO)联合肺功能检测指标诊断胸闷变异性哮喘(chest tightness variant asthma,CTVA)的价值。方法选取我院2015年1月至2019年12月收治的以胸闷作为唯一或主要临床表现的患儿92例,其中CTVA组31例,对照组61例。两组患儿行FeNO及肺功能联合检查,绘制受试者工作特征(receiver operating characteristic,ROC)曲线,比较各参数的诊断价值。结果CTVA组患儿FeNO值高于对照组(P<0.05),FEF50、FEF75及MMEF值均低于对照组患儿(P<0.05);FeNO值与FEF50、FEF75、MMEF值呈现负相关(r=-0.560,r=-0.559,r=-0.606,P<0.05);FeNO值诊断CTVA的AUC为0.870(P<0.05),敏感度为83.9%,特异度为83.6%,FeNO值和FEF75联合诊断CTVA的AUC最大(0.907,P<0.05),敏感度为85.2%,特异度为87.1%。结论FeNO值联合小气道功能检测可以提高对CTVA诊断的效能,可为CTVA的筛查及诊断提供有力的临床应用价值。 Objective To analyze the diagnostic value of fractional exhaled nitric oxide(FeNO)combined with pulmonary functional parameters in children with chest tightness variant asthma(CTVA).Methods From January 2015 to December 2019,a total of 92 children with chest tightness as the sole presenting manifestation were selected,including 31 cases in the CTVA group and 61 cases in the control group.FeNO and other pulmonary function parameters were measured,and it used receiver-operating characteristic(ROC)curve analysis to compare the sensitivity and specificity among the diagnostic parameters.Results The value of FeNO was significantly higher in the CTVA group than in the control group(P<0.05),and the value of MEF50,MEF75 and MMEF were significantly lower in the CTVA group than in the control group(P<0.05).The level of FeNO was negatively correlated with FEF50,FEF75 and MMEF(r=-0.560,r=-0.559,and r=-0.606,respectively,P<0.05).The AUC of FeNO diagnosing CTVA was 0.870(P<0.05),the sensitivity was 83.9%,and the specificity was 83.6%.The AU C of FeNO combined with FEF75 was the largest(0.907,P<0.05),and the sensitivity was 85.2%and the specificity was 87.1%.Conclusion FeNO combined with pulmonary function parameters has significant efficiency for the diagnosis of CTVA and can provide a powerful clinical application for CTVA screening and diagnosis.
