摘要
目的了解城市老年人群对家庭病床的需求,并分析其影响因素,为更好地发展和推广家庭病床模式提供依据。方法采用问卷调查,对武汉市某三甲医院病区298名60岁以上老年人及家中有60岁以上老年人家属的自身及家庭基本情况、患慢性病情况、2019年各项医疗费用及对家庭病床了解程度等进行调查。对数据进行一般描述性分析、2检验、多项Logistic回归分析。结果老年人接受家庭病床服务的意愿率为37.9%,表示不愿意的为16.8%,表示无所谓的为45.3%。通过多项Logistic回归分析得知,老年人是否患有慢性病、赡养老年人医疗费用、医院花费、社区卫生服务中心花费以及对家庭病床的了解程度等因素,对老年人群接受家庭病床服务的意愿有影响(P<0.05)。结论老年人对家庭病床服务的知晓程度相对较低,患有慢性病的老年人,医疗费用支出高、收入水平较低的家庭接受家庭病床服务意愿较高,社区医疗服务机构的水平影响老年人群接受家庭病床服务。
Objective To understand the needs of the urban elderly for family healthcare bed service and analyze its influencing factors to provide a basis for better development and promotion of family healthcare bed ser-vice.Methods A questionnaire survey was conducted among 298 elderly people over 60 years old and their family members in the ward of a tertiary grade-A hospital in Wuhan.The survey mainly investigated the basic information of themselves and their families,the status of chronic diseases,the medical expenses in 2019,and their understanding of family healthcare beds.Methods Such as general descriptive analysis,χ2 test,and multivariate logistic regression analysis were used to analyze the data.Results The willingness rate to accept family healthcare bed service was 37.9%,16.8%expressed unwillingness,and 45.3%expressed indifferent.Multiple logistic regression analysis showed that factors affecting the willingness of the elderly to receive family healthcare bed service(P<0.05)including:whether the elderly suffer from chronic diseases,the medical expenses of supporting the elderly,hospital expenses,community health service center expenses,and the degree of understanding of family healthcare bed ser-vice.Conclusion The elderly have relatively low knowledge of family healthcare bed service.Elderly people with chronic diseases,families with high medical expenses and lower income levels are more willing to accept healthcare service at home.The level of community medical service institutions affects the elderly to accept the family health care service.
