摘要
目的探讨突出型和脱垂游离型腰椎间盘突出症(lumbar disc herniation,LDH)髓核组织CD68表达差异及临床意义。方法本研究选择2019年1月~2020年1月于本院治疗的78例LDH患者,其中突出型48例,脱垂游离型30例。记录两组患者术前ODI指数以及腰腿疼痛VAS评分,术中摘除的髓核组织采用免疫组化染色检测CD68阳性表达率,采用ELISA检测TNF-α、IL-1表达情况,并分析CD68阳性表达率与ODI指数、VAS评分、TNF-α水平、IL-1水平的相关性。结果脱垂游离组腰痛VAS评分、腿痛VAS评分、ODI指数均显著高于突出组(P<0.05);脱垂游离组髓核组织CD68阳性表达率、TNF-α水平、IL-1水平显著高于突出组(P<0.05);LDH患者髓核组织CD68阳性表达率与TNF-α水平、IL-1水平、腰痛VAS评分、腿痛VAS评分、ODI指数均呈正相关关系(P<0.05)。结论脱垂游离型LDH的CD68阳性表达率显著高于突出型LDH,且CD68阳性表达率与功能障碍、疼痛症状呈正相关,与炎性因子表达呈正相关。
Objective To explore the difference and clinical significance of CD68 expression in the nucleus pulposus tissue of herniated and prolapsed free lumbar disc herniation(LDH).Methods Seventy-eight patients with LDH treated in our hospital from January 2019 to January 2020 were selected in the study,including 48 cases of prominent type and 30 cases of free type prolapse.They were set as the protrusion group and the prolapse free group.The preoperative ODI index and VAS scores of waist and leg pain in the two groups were recorded.The positive expression rate of CD68 in nucleus pulposus tissue removed during the operation was detected by immunohistochemical staining,and the levels of TNF-αand IL-1 were detected by ELISA.The correlation between CD68 positive expression rate and ODI index,VAS score,TNF-αlevel,IL-1 level were analyzed.Results The low back pain VAS score,leg pain VAS score and ODI index of the prolapse free group were significantly higher than those of the protruding group(P<0.05).The positive expression rate of CD68,the levels of TNF-αand IL-1 in the nucleus pulposus tissue of the prolapse free group were significantly higher than those of the protruding group(P<0.05).The positive expression rate of CD68 in the nucleus pulposus tissue of LDH patients was positively correlated with TNF-αlevel,IL-1 level,low back pain VAS score,leg pain VAS score and ODI index(P<0.05).Conclusion The positive expression rate of CD68 in prolapse free LDH is significantly higher than that of prominent LDH,and is positively correlated with dysfunction and pain symptoms,and positively correlated with the expression of inflammatory factors.
