摘要
目的探讨妊娠期孕妇巨细胞病毒(Cytomegalovirus, CMV)感染与Toll样受体9(TLR9)基因2848位点多态性及血清水平的关联性。方法选择海口市妇幼保健院妇产科2018年5月-2020年3月进行巨细胞病毒(CMV)筛查的阳性孕妇100例作为研究组,按照1∶1的比例,选择同期医院收治的CMV阴性孕妇100例作为对照组。采用聚合酶链式扩增反应(PCR)检测两组孕妇外周血中TLR9基因2848位点多态性,采用酶联免疫吸附法(ELISA)检测两组孕妇血清中TLR9水平,采用实时荧光定量聚合酶链式反应(RT-PCR)检测血液中CMV-DNA水平。结果两组孕妇TLR9基因2848位点GG、GA、AA基因型及G/A等位基因频率比较差异无统计学意义;活动性CMV感染孕妇TLR9 2848位点AA基因型频率(57.14%)高于潜伏性CMV感染孕妇(P<0.05),GG、GA基因型频率比较差异无统计学意义;活动性CMV感染组不同基因型TLR9、CMV-DNA水平总体差异具有统计学意义(P<0.05),AA型组TLR9、CMV-DNA水平最高,分别为(73.37±21.25)ng/ml、(1.98±0.41)Log copies/μl;活动性CMV感染孕妇TLR9与CMV-DNA水平成正相关关系(r=0.326,P=0.008)。结论 TLR9基因2848位点基因多态性与妊娠期巨细胞病毒易感性无关,但与病毒活动性有关,A等位基因为易感基因。
OBJECTIVE To explore the correlation between Toll like receptor 9(TLR9) gene 2848 locus polymorphism, serum level and cytomegalovirus(CMV) infection in pregnant women during pregnancy. METHODS A total of 100 pregnant women who were positive for screening of CMV and treated in obstetrics and gynecology department of Haikou Maternal and Child Health Care Hospital were assigned as the study group, meanwhile, 100 CMV-negative pregnant women who were treated in the hospital were chosen in a 1∶1 ration and set as the control group.The polymorphisms of TLR9 gene 2848 locus in peripheral blood were detected by using polymerase chain reaction(PCR), the serum TLR9 level of the two groups of pregnant women was determined by means of enzyme-linked immunosorbent assay, and the serum CMV-DNA level was detected with the use of real-time fluorescence quantitative polymerase chain reaction(RT-PCR). RESULTS There were no significant differences in GG, GA, AA genotypes and G/A allele frequency at 2848 locus of TLR9 gene between the two groups of pregnant women;the frequency of AA genotype at TLR9 2848 locus of the pregnant women with acute CMV infection was 57.14%, significantly higher than that of the pregnant women with latent CMV infection(P<0.05), and there were no significant differences in the frequencies of GG and GA genotypes. There were significant differences in the levels of TLR9 and CMV-DNA among the active CMV infection patients with different genotypes(P<0.05);the TLR9 level of the patients with AA genotype was the highest(73.37±21.25)ng/ml, the CMV-DNA level(1.98±0.41)copies/L.The TLR9 level of the pregnant women with active CMV infection was positively correlated with the CMV-DNA level(r=0.326,P=0.008). CONCLUSION The gene polymorphism at 2848 locus of the TLR9 gene is not associated with the susceptibility to CMV during the pregnancy but is associated with the activity of the virus.The A allele is a susceptible gene.
