摘要
目的研究联合使用利拉鲁肽和二甲双胍对2型糖尿病患者左心室舒张功能的影响,以及炎症因子与其降低心血管风险的关系。方法98例2型糖尿病患者为研究对象,最终46例患者符合入组条件,随机分为二甲双胍+利拉鲁肽组(治疗12周,24例)和二甲双胍+甘精胰岛素组(治疗24周,22例)。分析两组患者的临床生化指标和炎症因子水平。结果使用二甲双胍+利拉鲁肽在有效控制血糖的同时,升高了2型糖尿病患者超声心动图舒张早期二尖瓣血流峰值速度(E)/舒张晚期二尖瓣血流峰值速度(A)、二尖瓣环处舒张早期最大运动速度(Em),降低了E/Em及血清白细胞介素(IL)-1、IL-6水平,且炎症因子的降低与E/A、Em升高和E/Em降低具有显著的相关性(P<0.05)。使用二甲双胍+甘精胰岛素没有显著改善患者的左心室舒张功能与炎症。结论联合使用利拉鲁肽和二甲双胍可以有效降低2型糖尿病患者心血管疾病风险,机制可能与其抗炎作用有关。
Objective To study the effect of liraglutide combined with metformin on left ventricular diastolic function,and the correlation between inflammatory factors and the reduction of cardiovascular risk.Methods A total of 98 patients with type 2 diabetes mellitus were taken as the subjects of the study.The final 46 patients met the enrollment criteria were randomly divided into the metformin+liraglutide group(treatment for 12 weeks,24 cases)and the metformin+insulin glargine group(treatment for 24 weeks,22 cases).The clinical biochemical indexes and inflammatory factors were compared between the two groups.Results The use of metformin+liraglutide can effectively control blood glucose while increasing the peak mitral flow velocity in early diastole to that during atrial contraction(E/A)of patients with type 2 diabetes mellitus,maximum early diastolic velocity of mitral annulus(E_m),and reducing the levels of E/E_m and serum interleukin(IL)-1,IL-6.The decrease of inflammatory factors was significantly correlated with the increase of E/A,Em and the decrease of E/E_m(P<0.05).Metformin and insulin glargine did not significantly improve left ventricular diastolic function and inflammation.Conclusion Combination of liraglutide and metformin can effectively reduce the risk of cardiovascular disease in patients with type 2 diabetes mellitus,and its mechanism may be related to its anti-inflammatory effect.
