摘要
患者女,48岁,发现头顶部两处蓝黑色丘疹8年余。皮肤科情况:头顶左右两侧各见大小约1 cm×0.8 cm、1 cm×0.5 cm大小蓝黑色丘疹,表面不规则,与周围组织分界清楚,无红肿及破溃。皮肤镜示:皮损处界限清楚,可见蓝黑色丘疹,大致呈均质模式。两处皮损组织病理示:表皮大致正常,真皮中上部可见呈梭形或分支状的黑素细胞,细胞内大量黑素颗粒,多数细胞与表皮平行或交织排列。免疫组织化学染色:HMBH45(+)、SOX10(+)。诊断:头皮普通型蓝痣。
A 48-year-old female had two blue-black papules on the top of her head for more than eight years.Physical examination revealed:blue-black papules with a size of about 1 cm×0.8 cm and 1 cm×0.5 cm were on the left and right sides top of the head.The surface was irregular,and the boundary with surrounding tissues was clear,without redness,swelling or rupture.Dermatoscopy showed that the skin lesions were clearly demarcated,and blue-black papules were seen in a roughly homogeneous pattern.Pathological results of two lesions showed that the epidermis was generally normal.In the middle and upper part of the dermis,there were spindle shaped or branching melanocytes with a large number of melanin granules.Most of the cells were parallel or interwoven with the epidermis.Immunohistochemical staining showed HMBH45(+),SOX10(+).The diagnosis of nevus ceruleus was made.
