摘要
目的系统评价3D打印导板辅助置钉技术与传统置钉技术治疗寰枢椎不稳有效性与安全性的差异。方法计算机检索知网、万方、维普、中国生物医学文献服务系统、Cochrane Library、PubMed、Embase等数据库,搜集有关3D打印导板辅助置钉技术(导板组)与传统置钉技术(传统组)治疗寰枢椎不稳的临床对照研究,并手工检索相关论文。筛选文献、提取资料并评价文献质量后,采用Stata/SE 12.0软件进行Meta分析。结果共纳入9个队列研究。Meta分析结果显示,导板组的手术时间、术中出血量、置钉准确率、并发症发生率以及透视次数均优于传统组(均P<0.05);但两种技术在术后视觉模拟评量表评分和日本骨科协会评分方面,差异均无统计学意义(均P>0.05)。结论与传统置钉技术相比,3D打印导板辅助置钉技术治疗寰枢椎不稳具有手术时间短、术中出血量少、置钉准确率高、并发症发生率低以及透视次数少等优势。
Objective To systematically evaluate the difference in effectiveness and safety between 3D printing navigational template-assisted screw placement technique and traditional screw placement technique in the treatment of atlantoaxial instability.Methods A computer retrieval was conducted on the databases including CNKI,Wanfang Data,VIP,CBM,The Cochrane Library,PubMed and Embase to collect clinical controlled trials regarding 3D printing navigational template-assisted screw placement technique(template group)and traditional screw placement technique(traditional group)in the treatment of atlantoaxial instability.Meanwhile,a manual retrieval of relevant articles was performed.After literature screening,data extraction and quality evaluation of the literature,a Meta-analysis was carried out by using Stata/SE 12.0 software.Results In total,nine cohort studies were included.The Meta-analysis results showed that the template group was superior to the traditional group in terms of operation time,intraoperative blood loss,accuracy of screw placement,incidence rate of complications,and number of intraoperative fluoroscopy(all P<0.05),but there was no statistically significant difference between the two techniques in postoperative Visual Analogue Scale score or Japanese Orthopaedic Association score(all P>0.05).Conclusion Compared with traditional screw placement technique,3D printing navigational template-assisted screw placement technique in treatment of atlantoaxial instability exhibits the advantages of shorter operation time,less intraoperative blood loss,higher accuracy of screw placement,lower incidence rate of complications and minor number of fluoroscopy.
