摘要 目的探讨改良微量肉汤药敏实验(荧光法)应用效果及可靠性。方法通过改良微量肉汤药敏实验(荧光法)、VITEK2全自动细菌鉴定仪、微量肉汤稀释法分别对标准菌株金黄色葡萄球菌ATCC29213和大肠埃希菌ATCC25922进行三种不同抗生素(金黄色葡萄球菌ATCC29213:苯唑西林、利奈唑胺和环丙沙星;大肠埃希菌ATCC25922:庆大霉素、环丙沙星和亚胺培南)的药敏实验,并对5株临床分离金黄色葡萄球菌和大肠埃希菌分别进行4种不同抗生素(金黄色葡萄球菌:苯唑西林、利奈唑胺、青霉素和红霉素;大肠埃希菌:亚胺培南、头孢曲松、哌拉西林和妥布霉素)的药敏实验,检测最低抑菌浓度(MIC)并记录检测周转时间(turn-around time,TAT),比较三种方法检测结果的一致性。结果改良微量肉汤药敏实验(荧光法)与VITEK2全自动细菌鉴定仪、微量肉汤稀释法检测标准菌株和临床分离菌株的MIC基本一致(Cohen’skappa值为1.000,P=0.083),即三种抗生素敏感性实验结果具有高度吻合性;改良微量肉汤药敏实验(荧光法)对金黄色葡萄球菌的TAT(约10 h)均低于VITEK2全自动细菌鉴定仪(约17 h)和微量肉汤稀释法(约22 h)(P均<0.05)。结论改良微量肉汤药敏实验(荧光法)较VITEK2全自动细菌鉴定仪更快捷,较微量肉汤稀释法更省时,并可通过进一步优化为今后的临床检验工作提供全新的快速药敏检测方法。 Objective To explore the effectiveness and reliability of application of modified micro broth drug sensitivity test(fluorescence method).Methods The standard strains of Staphylococcus aureus ATCC29213 and Escherichia coli ATCC25922 were subjected to three different antibiotics(Staphylococcu saureus ATCC29213:oxacillin,linezolid and ciprofloxacin.Escherichia coli ATCC25922:gentamicin,ciprofloxacin and imipenem)were carried out by the modified micro broth susceptibility test(fluorescence method),VITEK2 automatic bacterial identification instrument and micro broth dilution method.Five clinical isolates of Staphylococcus aureus and Escherichia coli were tested with four different antibiotics(Staphylococcus aureus:oxacillin,linezolid,penicillin and erythromycin.Escherichia coli:imipenem,ceftriaxone,piperacillin and tobramycin),the MIC was detected and TAT was recorded,and the consistency of the three methods were compared.Results The MIC of standard strains and clinical isolates detected by the modified micro broth susceptibility test(fluorescence method)was basically consistent with VITEK2 automatic bacterial identification instrument and micro broth dilution method(Cohen’s kappa value was 1.000,P=0.083),that means the results of three kinds of antibiotic sensitivity tests were highly consistent.The TAT(about 10 h)of the modified micro broth susceptibility test(fluorescence method)was significantly lower than that of the VITEK2 automatic bacterial identification instrument(about 17 h)and the micro broth dilution method(about 22 h)(P all<0.05).Conclusion Compared with VITEK2 automatic bacterial identification instrument,the modified micro broth susceptibility test(fluorescence method)is faster,and it is more time-saving than the micro broth dilution method.Moreover,through further optimization,it can provide a new rapid drug sensitivity detection method for the future clinical test work.
