摘要
目的:分析六氟化硫微泡致过敏性休克的发生规律,为安全用药提供参考。方法:检索2001年1月~2019年12月国内外数据库关于六氟化硫微泡致过敏性休克个案报道文献,进行总结分析。结果:共筛选出个案报道11篇,涉及13例患者。其中,男性8例,女性5例;患者年龄≥60岁4例,其中最小年龄34岁,最大年龄80岁;有高血压病史2例、糖尿病史1例、心力衰竭病史1例、肝硬化病史3例。六氟化硫微泡致过敏性休克发生时间多在用药5 min内,临床表现主要为呼吸困难、意识丧失、血压骤降、周身皮肤潮红;4例发生在用药过程中;过敏性休克发生后给予吸氧、抗过敏等对症治疗后,有6例治愈、7例缓解,无死亡病例。结论:有心血管基础疾病史患者静脉注射六氟化硫微泡时可能更易发生过敏性休克,临床应用时应提高警惕。
Objective:To analyze the occurrence regularity of anaphylactic shock caused by sulfur hexafluoride microbubbles,and to provide references for safety use of drugs.Methods:The case report literatures of anaphylactic shock caused by sulfur hexafluoride microbubbles in domestic and foreign databases from January 2001 to December 2019 were searched,analyzed and summarized.Results:A total of 11 case reports were screened,involving 13 patients,8 males and 5 females among them.4 patients≥60 years old,the youngest age was 34 years old,the oldest age was 80 years old;2 cases of hypertension,1 case of diabetes,1 case of heart failure and 3 cases of liver cirrhosis.Anaphylactic shock caused by sulfur hexafluoride microbubbles usually occurs within 5 min of medication.The clinical manifestations are dyspnea,loss of consciousness,sudden drop in blood pressure,and skin flushing all over the body.4 cases occurred during medication.After the occurrence of anaphylactic shock,the patients were given symptomatic treatment such as oxygen inhalation and anti-allergy,6 cases were cured,7 cases improved,and no deaths occurred.Conclusion:Patients with a history of basic cardiovascular diseases may be more prone to anaphylactic shock when in-travenously injected with sulfur hexafluoride microbubbles,and should be noticed in clinical application.
