摘要
目的检测妊娠糖尿病(GDM)患者血清中胰岛素受体底物-1(IRS-1)的表达水平,同时分析其与胰岛素抵抗(IR)的相关性。方法选择该院2019年5月-2020年5月收治的分娩期孕妇86例,根据OGTT试验结果分为GDM组44例和正常糖耐量组(NGT组)42例,使用ELISA的方法检测两组患者血清IRS-1的表达水平,同时检测患者空腹血糖(FPG)、餐后1 h血糖(1 hPG)、餐后2 h血糖(2 hPG)、空腹胰岛素(FINS)、空腹C肽、胰岛素抵抗指数(HOMA-IR)、糖化血红蛋白(HbA1c)指标。结果GDM组患者FPG、1 hPG、2 hPG、FINS、空腹C肽、HOMA-IR、HbA1c水平均高于NGT组,而IRS-1水平低于NGT组,差异有统计学意义(P<0.05)。同时经过Pearson相关性分析显示,GDM组患者IRS-1表达水平与患者HOMA-IR、FPG、1 hPG、2 hPG、FINS、HbA1c水平呈明显负相关(P<0.05)。结论GDM孕妇存在明显的IR,其与患者血清IRS-1的表达呈负相关,因此低IRS-1在GDM的发生发展中起到一定的作用,这一结果为将来GDM的治疗提供了依据。
Objective To detect the expression level of insulin receptor substrate-1(IRS-1)in the serum of patients with gestational diabetes mellitus(GDM),and to analyze its correlation with insulin resistance(IR).Methods A total of 86 pregnant women during childbirth admitted to the hospital from May 2019 to May 2020 were selected.According to the OGTT test results,they were divided into 44 cases in the GDM group and 42 cases in the normal glucose tolerance group(NGT group).The sera of the two groups were detected by ELISA.The expression level of IRS-1,and simultaneously detect the patient’s fasting blood glucose(FPG),1 h postprandial blood glucose(1 hPG),2 h postprandial blood glucose(2 hPG),fasting insulin(FINS),fasting C-peptide,insulin resistance index(HOMA-IR),glycosylated hemoglobin(HbA1c)indicators.Results The levels of FPG,1 hPG,2 hPG,FINS,fasting C peptide,HOMA-IR,and HbA1c in the GDM group were higher than those in the NGT group,while the IRS-1 level was lower than that in the NGT group.The differences were statistically significant(P<0.05).At the same time,Pearson correlation analysis showed that the expression level of IRS-1 in the GDM group was significantly negatively correlated with the levels of HOMA-IR,FPG,1 hPG,2 hPG,FINS,and HbA1c,and the difference was statistically significant(P<0.05).Conclusion There is obvious IR in pregnant women with GDM,which is negatively correlated with the expression of serum IRS-1.Therefore,low IRS-1 plays a certain role in the occurrence and development of GDM.This result provides a basis for future treatment of GDM.
