摘要
目的:探讨合理情绪疗法联合基于循证的预防性护理在靶向联合免疫治疗肿瘤患者中的应用效果。方法:选取2019年1月1日~2020年2月1日治疗的104例靶向联合免疫治疗肿瘤患者为研究对象,采用随机数字表法分为观察组53例和对照组51例。对照组采用常规护理,观察组采用合理情绪疗法联合基于循证的预防性护理;比较两组护理前后心理状态[采用焦虑自评量表(SAS)、抑郁自评量表(SDS)]、生活质量[采用生活质量综合评定问卷(GQOL-74)]、营养指标[包括血清白蛋白(ALB)、血红蛋白(Hb)]及不良反应发生情况。结果:护理后,两组SAS评分、SDS评分低于护理前(P<0.05),且观察组低于对照组(P<0.05);护理后,两组GQOL-74中的社会社交情况、心理状态、躯体功能、生活质量评分均高于护理前(P<0.05),且观察组高于对照组(P<0.05);护理后,两组ALB、Hb浓度均高于护理前(P<0.05),且观察组Hb浓度高于对照组(P<0.05);观察组脱发、皮肤瘙痒、骨髓抑制发生率低于对照组(P<0.05)。结论:合理情绪疗法联合基于循证的预防性护理可改善靶向联合免疫治疗肿瘤患者的心理状态、生活质量、营养状况,降低其不良反应发生率。
Objective:To explore the application effect of rational-emotive therapy(RET)combined with evidence-based preventive care in the targeted combined immunotherapy for cancer patients.Methods:A total of 104 cancer patients undergoing targeted combined immunotherapy during January 1,2019 to February 1,2020 were enrolled and divided into the observation group with 53 cases and the control group with 51 cases by the random number table method.The observation group was given RET combined with evidence-based preventive care,while the control group was given routine care.Comparison was made before and after nursing,on psychological status by Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS),quality of life by Generic Quality of Life Inventory-74(GQOL-74),the levels of nutritional indexes[serum albumin(ALB),hemoglobin(Hb)],and the occurrence of adverse reactions in both groups.Results:After nursing,the SAS and SDS scores in both groups were lower than those before nursing(P<0.05),and lower in the observation group than those in the control group(P<0.05);and scores of social contact,psychological status,physical function and quality of life in GQOL-74 were all higher than those in the control group(P<0.05).And higher in the observation group than those in the control group(P<0.05).After nursing,the ALB and Hb concentrations were higher than those before nursing(P<0.05),and the concentratipon of Hb was higher in the observation group than that in the control group(P<0.05);The incidence of hair loss,skin itching and bone marrow suppression was lower in the observation group than that in the control group(P<0.05).Conclusion:RET combined with evidence-based preventive care can improve psychological status and quality of life in cancer patients undergoing targeted combined immunotherapy and nutritional status,and reduce the occurrence of adverse reactions.
