摘要
目的:探讨基于老年综合评估(CGA)的聚焦解决模式对老年冠心病(CAD)患者孤独感、抗逆力及正念水平的影响。方法:选取2018年1月1日~2020年12月31日收治的124例老年CAD患者作为研究对象,采用随机数字表法分为对照组61例和干预组63例,均给予相应积极治疗,对照组给予常规护理,干预组在常规护理基础上给予基于CGA的聚焦解决模式护理;比较两组干预前、干预3个月后孤独感[采用孤独感测评量表(UCLA)]、抗逆力[采用抗逆水平量表(CD-RISC)]、正念水平[采用正念五因素量表(FFMQ)]、应对方式[采用老年应对方式问卷(WOCS)]、自我管理行为[采用冠心病自我管理行为量表(CSMS)]。结果:干预3个月后,两组CD-RISC各项评分、FFMQ各因素评分、WOCS中积极应对(PC)维度评分、CSMS各方面评分均较干预前上升(P<0.05),且干预组高于同期对照组(P<0.05,P<0.01);干预3个月后,两组UCLA各项评分、WOCS中消极应对(NC)维度评分均较干预前下降(P<0.05),且干预组低于同期对照组(P<0.05,P<0.01)。结论:将基于CGA的聚焦解决模式用于老年CAD患者护理中取得满意效果,可有效缓解其孤独感,改善抗逆力与正念水平,进而促进积极应对心理,并提高自我管理行为,有利于患者预后。
Objective:To explore the effect of solution focused approach based on comprehensive geriatric assessment(CGA)on loneliness,resilience and mindfulness level in elderly patients with coronary artery disease(CAD).Methods:A total of 124 elderly patients with CAD admitted during January 1,2018 to December 31,2020 were selected as the research subjects,and they were divided by the random number table method into the control group with 61 cases and the intervention group with 63 cases,corresponding active treatment was given.the control group was given routine nursing,and the intervention group was given CGA-based solution focused approach nursing.The loneliness[University of California Loneliness Scale(UCLA)],resilience[Connor-Davidson Resistance Scale(CD-RISC)],mindfulness level[Five Facet Mindfulness Questionnaire(FFMQ)],coping styles[Ways of Coping Scale(WOCS)]and self-management behaviors[Coronary Self-Management Scale(CSMS)]were compared between the two groups before intervention and after 3 months of intervention.Results:After 3 months of intervention,the CD-RISC scores,FFMQ scores,scores of positive coping(PC)dimension of WOCS and CSMS scores in both groups increased compared with those before intervention(P<0.05),and the scores in the intervention group were significantly higher than those in the control group during the same period(P<0.05,P<0.01).The scores of items of UCLA and scores of negative coping(NC)dimension of WOCS in the two groups decreased compared with those before intervention(P<0.05),and the scores in the intervention group were lower than those in the control group during the same period(P<0.05,P<0.01).Conclusion:CGA-based solution focused approach can achieve ideal results in the nursing of elderly patients with CAD,and it can effectively alleviate the loneliness,improve the resilience and mindfulness level,and promote positive coping and self-management behaviors,and it is beneficial to the prognosis and rehabilitation of patients.
