影像技术对卒中微血管状态及卒中短期恶化和近期复发的评估价值研究分析

摘要 目的:分析影像技术对卒中微血管状态及卒中短期恶化和近期复发的评估价值。方法:选取2019年2月-2020年2月来笔者所在医院就诊的60例缺血性脑卒中患者作为研究对象,对所有患者实施头颈部磁共振血管成像(MRA)检查与CT检查,以患者住院期间卒中恶化与3个月内卒中复发为终点事件,分析不同预后患者及相应的MRA、CT检查参数。结果:60例患者中30例短期恶化,其中15例出现近期复发。短期恶化患者与非短期恶化患者性别比例比较差异无统计学意义(P>0.05),短期恶化患者年龄、多发颅内血肿患者占比、外血管狭窄>50%患者占比、颅内动脉狭窄>50%患者占比、最大斑块横断面面积、出血量、NIHSS评分均高于非短期恶化患者,差异均有统计学意义(P<0.05)。近期复发与近期未复发患者性别比例比较差异无统计学意义(P>0.05),近期复发患者年龄、多发颅内血肿患者占比、外血管狭窄>50%患者占比、颅内动脉狭窄>50%患者占比、最大斑块横断面面积、出血量、NIHSS评分均高于非短期恶化患者,差异均有统计学意义(P<0.05)。经多因素分析证实年龄≥65岁、外血管狭窄、颅内动脉狭窄、出血量多、NIHSS评分高为短期恶化及近期复发的高危因素,差异均有统计学意义(P<0.05)。结论:卒中患者加强MRA与CT影像学检查,能够根据获取微血管状态及动脉血管狭窄等相关信息,为卒中短期恶化及近期复发提供依据,便于尽早采取措施,减少卒中的近期复发,延长患者生存时间。 Objective:To analyze the evaluation value of imaging technology for stroke microvascular status,short-term stroke deterioration and shortterm recurrence.Method:Sixty patients with ischemic stroke who came to our hospital from February 2019 to February 2020 were selected as the research subjects,and all patients underwent head and neck magnetic resonance angiography(MRA)and CT examination,with stroke worsening during hospitalization and stroke recurrence within 3 months as the endpoint events.Patients with different prognosis and corresponding MRA and CT examination parameters were analyzed.Result:Of the 60 patients,30 had a short-term deterioration and 15 had a recent recurrence.There was no significant difference in sex ratio between short-term deterioration patients and non-short-term deterioration patients(P>0.05),the age,the proportion of patients with multiple intracranial hematomas,the proportion of patients with external vascular stenosis>50%,the proportion of patients with intracranial artery stenosis>50%,the maximum plaque crosssectional area,the amount of blood loss,and the NIHSS score of patients with short-term deterioration were all higher than those of patients with non-shortterm deterioration,the differences were statistically significant(P<0.05).There was no significant difference in sex ratio between patients with recent recurrence and those without recent recurrence(P>0.05),the age,the proportion of patients with multiple intracranial hematoma,the proportion of patients with external vascular stenosis>50%,the proportion of patients with intracranial artery stenosis>50%,the maximum plaque cross-sectional area,the amount of blood loss,and the NIHSS score of patients with recent recurrence were all higher than those of patients with non-short-term deterioration,the differences were statistically significant(P<0.05).By multivariate analysis,it was confirmed that age≥65 years old,external vessel stenosis,intracranial artery stenosis,high blood loss,and high NIHSS score were the high risk factors fo
