摘要
医疗决策是临床活动中的重要部分,但仍然面临很多问题,如医患社会角色的差异、医患对躯体和疾病的认知差异、医患对疾病归因机制的差异和医患道德情感的缺失,这些问题都阻碍了临床决策的有效实施。叙事医学的“主体间性”“独特性”“因果联系/偶然性”“伦理性”四大特征为提升医疗决策质量提供了有价值的启示和新的解决思路。叙事医学作为推进医学人文的手段,在医疗决策中践行叙事医学有利于医患之间的深度沟通和共情,最终制定高质量的决策并提升患者满意度。但叙事医学的发展在我国仍处于起步阶段,需要学校、医院等各方共同努力推动其临床实践。
Making medical decision is an important part of medical activities,but there are many problems in the field of medical decision-making,such as the difference of doctor-patient social role,the differences of doctor-patient cognition of body and disease,the differences of doctor-patient cognition of attribution mechanism and the lack of moral emotions between doctors and patients.All these problems hinder the effective implementation of clinical decision-making.The four characters of narrative medicine,which including“inter-subjectivity”“uniqueness”“causal connection/contingency”and“ethics”,provide the valuable enlightenment and new solutions for improving the quality of medical decisions-making.As the way of promoting medical humanities,practicing of narrative medicine is conducive to deep communication and empathy between doctors and patients,and making high-quality decisions and improving the patients’satisfactions finally in medical decision-making.However,narrative medicine is at the initial stage in China,its practice requires the joint efforts of colleges,hospitals and related groups.
