摘要
目的了解2018年张家口市关于克山病的病情及发展趋势及各型克山病构成,根据病情调查结果进行控制、消除、评价及判定,为下一步防治提供科学依据。方法根据张家口市克山病的分布情况,选定张北县1个乡3个病区村作为调查点。对调查点居民进行临床查体和心电图描记;对疑似克山病患者拍摄胸部正位X线片,测量心胸比率,进行心脏彩超检查。结果本次调查于2018年对3个病区村共检查832人,检出慢型克山病患者1例、潜在型克山病患者10例,检出率分别为0.12%(1/832)、1.20%(10/832),未检出急型和亚急型克山病患者。共描记心电图832份,其中心电图异常249例,异常率为29.93%,异常项344项,异常心电图以窦性心动过缓、T波改变为主。在11例克山病患者中,1例慢型克山病心电图表现为完全性右束支传导阻滞(complete right bundle branch block,CRBBB)、左前分支传导阻滞(left anterior hemiblock,LAH)和Q-T间期延长,另外10例潜在型克山病患者心电图主要表现为CRBBB,有异常项18项,平均每人异常项1.8个。对11例克山病患者拍摄胸部正位X线片和心脏彩色B超检查进行分析,X线片检查出心脏增大者2例,检出率为18.18%,其中轻度增大1例、重度增大1例;在心脏彩色B超检查中,有3例异常改变,异常率为27.27%。结论张家口市调查点克山病病情稳定,无急型和亚急型患者,无慢型克山病新发患者,符合克山病消除标准。但仍应继续采取综合防控措施,巩固防治成果。
Objective To understand the condition and development trend of Keshan disease in Zhangjiakou City in 2018,and the composition of various types of Keshan diseases,and evaluate,judge and eliminate according to the investigation results of disease,to provide scientific basis for the next prevention and control.Methods According to the distribution of Keshan disease in Zhangjiakou City,3 wards and villages in 1 township in Zhangbei County were selected as the investigation locale.Clinical physical examination and electrocardiography were carried out for residents in the survey sites;chest X-ray examinations were taken for suspected Keshan disease patients,the cardiothoracic ratio was measured,and cardiac color Doppler ultrasound was carried out at the same time.Results A total of 832 people were examined in 3 wards and villages in 2018,1 cases of chronic Keshan disease and 10 cases of potential Keshan disease were detected,the postive rates of 0.12%(1/832) and 1.20%(10/832) respectively.No cases with acute or sub-acute Keshan disease were detected.A total of 832 electrocardiograms were recorded,of which 249 were abnormal.The abnormal rate was 29.93%,and 344 items were abnormal.The abnormal ECG mainly showed sinus bradycardia and T wave changes.Among the 11 cases of Keshan disease,1 case of chronic Keshan disease showed(complete right bundle branch block,CRBBB),left anterior hemiblock(LAH) and Q-T interval prolongation,another 10 potential Keshan disease patients mainly showed CRBBB,with18 abnormal items,an average of 1.8 abnormal items per person.Eleven patients with Keshan disease were analyzed by chest X-ray and color B-ultrasound.Two patients with enlarged hearts were found by X-ray,the detection rate was 18.18%.Among them,1 case was slightly enlarged,and 1 case was severely enlarged.There were 3 cases of abnormal changes,the abnormal rate was 27.27%.Conclusion There are no acute,subacute patients or no new cases of chronic Keshan disease in Zhangjiakou,which meets the elimination standard of Keshan disease.Howeve
