摘要 目的探讨CT增强中两种注射方式下碘造影剂对肾功能的影响。方法60例行CT增强扫描的患者,随机分为甲组和乙组,每组30例。两组均采用频率较高的低渗造影剂碘海醇,乙组采用双筒注射,甲组采用单筒注射。比较两组造影剂肾病(CIN)发生情况及检查前后肌酐水平。结果甲组中4例发生CIN,发生率为13.33%(4/30);乙组中5例发生CIN,发生率为16.67%(5/30)。两组患者CIN发生率比较,差异无统计学意义(χ^(2)=0.131,P=0.718>0.05)。两组患者检查前和检查后1、3、7 d的肌酐值水平比较差异无统计学意义(P>0.05);两组患者检查后1、3、7 d的肌酐值水平与检查前比较差异无统计学意义(P>0.05)。结论CT增强中经单筒或双筒注射碘造影剂对肾功能的影响均较小,应结合患者实况选用适宜的注射方式。 Objective To discuss the effect of iodine contrast agent on renal function under two injection modes during CT enhancement.Methods A total of 60 patients undergoing CT enhancement scanning were randomly divided into group A and group B,with 30 cases in each group.Both groups used the more frequent hypotonic contrast agent iohexol.Group B used double-barrelled syringe,and group A used singlebarrelled syringe.The incidence of contrast-induced nephropathy(CIN)and creatinine level before and after examination were compared between the two groups.Results There were 4 cases of CIN in group A,accounting for 13.33%(4/30),and 5 cases of CIN in group B,and accounting for 16.67%(5/30).There was no statistically significant difference in CIN incidence between the two groups(χ^(2)=0.131,P=0.718>0.05).There was no statistically significant difference in creatinine level before examination and 1,3,and 7 d after examination between the two groups(P>0.05).1,3,and 7 d after examination,there was no statistically significant difference in creatinine level compared with that before examination(P>0.05).Conclusion Both single and double-barrelled injection of iodine contrast agent during CT enhancement have little effect on renal function,so the appropriate injection method should be selected in combination with the actual situation of the patients.
