摘要
目的探讨增强CT扫描中应用含碘造影剂对老年患者肾功能的影响及口服水化治疗的预防效果。方法回顾性选取2016年8月至2018年8月江苏省张家港市中医医院收治的行增强CT扫描的老年患者120例,依据治疗方法分为两组:一组口服水化治疗组(水化组,n=60),一组非口服水化治疗组(非水化组,n=60),统计分析两组患者的尿β2微球蛋白、血胱抑素C、肌酐水平、造影剂肾病发生情况。结果水化组患者检查前后的尿β2微球蛋白、血胱抑素C、肌酐水平之间差异均无统计学意义(P>0.05),非水化组患者检查后的尿β2微球蛋白、血胱抑素C水平均显著高于检查前(P<0.05),但检查前后的血肌酐水平之间差异无统计学意义(P>0.05);检查前两组患者的尿β2微球蛋白、血胱抑素C、肌酐水平之间差异均无统计学意义(P>0.05),检查后水化组患者的尿β2微球蛋白、血胱抑素C水平均显著低于非水化组(P<0.05),但两组患者的血肌酐水平之间差异无统计学意义(P>0.05)。水化组患者的造影剂肾病发生率1.7%(1/60)明显低于非水化组15.0%(9/60),差异有显著性(χ^2=11.14,P<0.05)。结论增强CT扫描中应用含碘造影剂会损害老年患者肾功能,口服水化治疗能够有效预防造影剂肾病的发生。
Objective To explore the effect of iodine-containing contrast medium on renal function in elderly patients with enhanced CT scanning and the preventive effect of oral hydration therapy.Methods One hundred and twenty elder patients underwent enhanced CT scanning in this hospital during August 2016 to August 2018 were retrospectively selected and they were divided into two groups by random table method:Patients in Group one were treated with oral hydration(hydration group,n=60)and patients in another group were treated with non-oral hydration(non-hydration group,n=60).The urinary level of beta-2 microglobulin,serum levels of cystatin C and creatinine and the incidence of contrast-induced nephropathy in these two groups were statistically analyzed.Results There was no significant difference in urinary levels of beta-2 microglobulin,cystatin C and serum levels of creatinine before and after examination in hydration group(P>0.05).The urinary levels of beta-2 microglobulin and cystatin C in non-hydration group were significantly higher than those before examination(P<0.05),but there was no significant difference in serum level of creatinine before and after examination(P>0.05).There was no significant difference in urinary levels of beta 2 microglobulin,cystatin C and seerum level of creatinine between these two groups(P>0.05).After examination,the levels of urinary beta 2 microglobulin and cystatin C in hydration group were significantly lower than those in non-hydration group(P<0.05),but there was no significant difference in serum level of creatinine between these two groups(P>0.05).The incidence of contrast-induced nephropathy in patients of hydration group was 1.7%(1/60),it was lower than those of non-hydration group(15.0%(9/60),with significant statistical difference(χ2=11.14,P<0.05).Conclusion The application of iodine-containing contrast media in enhanced CT scanning can impair the renal function in elderly patients,oral hydration therapy can effectively prevent the occurrence of contrast-induced nephropathy.
