摘要
目的:分析慢性光化性皮炎(CAD)患者与长波紫外线(UVA)、中波紫外线(UVB)的关系,以及最小红斑量(MED)的影响因素。方法:以SUV1000型日光紫外线模拟器作为照射光源,比较330例CAD患者不同性别、年龄、病程及不同季节受试者UVA-MED值及UVB-MED值。结果:男性患者UVB-MED值明显低于女性MED值[(45.23±21.01)mJ/cm^(2)比(48.50±22.59)mJ/cm^(2),t=2.79,P=0.43]。年龄大于50岁组UVA-MED值和UVB-MED值明显低于同性别19~29岁组和30~50岁组(t值分别为3.12、1.03、3.03、1.32,P值均<0.05)。病程为12~36个月和大于36个月组UVA-MED值和UVB-MED值均明显低于同性别病程小于12个月组(t值分别为2.25、2.81、2.60、2.34,P值均<0.05)。男性CAD患者夏季UVA-MED值均低于春季、秋季MED值(t值分别为5.46、3.21,P值均<0.05),但与冬季相比差异无统计学意义(t=3.67,P=0.58)。男性和女性夏季UVB-MED值均明显低于春、秋和冬季(P值均<0.05)。结论:CAD的发病与UVA、UVB的照射有关,CAD患者的性别、年龄、病程以及季节变化等因素对MED值的检测有一定影响。
Objective:To explore the minimal erythema dose for UVA and UVB radiation in patients with chronic actinic dermatitis(CAD).Methods:SUV1000 type UV simulator was used to measure the minimal erythema dose(MED)value of UVA and UVB radiation in 330 CAD patients with different sex,age,length of course,and season.Results:Compared with female patients[(48.50±22.59)mJ/cm^(2)],male patients[(45.23±21.01)mJ/cm^(2)]had significantly lower MED value of UVB(t=2.79,P=0.43).Compared with patients>50 years old,patients with 19 to 29 years old and 30 to 50 years old had much higher MED value for UVA and UVB(t=3.12,1.03,3.03,1.32,respectively,all P<0.05).For the same gender,patients with a shorter course of fewer than 12 months had higher MED value for UVA and UVB than patients with a longer course(t=2.25,2.81,2.60,2.34,respectively,all P<0.05).The UVA-MED value of male CAD patients in summer was lower than those in spring and autumn(t=5.46,3.21,respectively,both P<0.05),while there was no significant difference when compared with the value in winter(t=3.67,P=0.58).The UVB-MED value was significantly lower in the summer than the values in other seasons for both male and female patients(all P<0.05).Conclusion:UVA and UVB play an important role in the pathogenesis of CAD.Sex,age,course of the disease,and seasonal variation are important factors of UVA-and UVB-MED in patients with CAD.
