摘要
目的:分析对牛乳蛋白过敏的婴儿湿疹患者在生长发育过程中特异性牛乳蛋白sIgE、血清总IgE和皮损表现的变化。方法:回顾性分析2015年7月至2020年7月57例严重婴儿湿疹患儿,观察期间(30个月)食用乳蛋白配方奶粉(氨基酸配方),严格规避牛乳蛋白及其制品,于入组开始和结束时评估患儿皮损积分,检测患儿血清牛乳蛋白sIgE和总IgE表达水平。结果:观察结束时,患儿皮损SCORAD积分从入组时的(56.98±10.24)分下降至(6.17±5.03)分,差异有统计学意义(t=29.08,P<0.01);牛乳蛋白sIgE值从入组时的(16.50±9.87)kU/L下降至(2.05±1.70)kU/L,差异有统计学意义(t=11.04,P<0.01);血清总IgE较同龄儿童正常值高限升高倍数从(11.99±5.35)倍下降至(4.50±1.08)倍,差异有统计学意义(t=10.51,P<0.01);45例患儿转常规奶粉成功,平均用时4.57个月。结论:大部分对牛乳蛋白过敏的严重湿疹患儿,经过正确治疗以及长时间规避牛乳蛋白后,其症状会减轻或消退,体内特异性牛乳蛋白sIgE水平和血清总IgE下降。
Objective:To study the changes of milk-specific IgE and total serum IgE in infants with severe eczema induced by milk allergy at the different ages.Methods:Totally 57 infants with severe eczema induced by milk allergy were recruited.Both milk-specific IgE and serum IgE were measured,and the degrees of lesion severity were assessed at the beginning and end of the study.The study period was 30 months.Participants were required to avoid milk consumption during the investigation.Results:Among 57 patients,the average SCORAD score at the end of the observation was significantly lower than that at the beginning of the observation(6.17±5.03 vs 56.98±10.24,t=29.08,P<0.01).The value of milk-specific IgE at the end of the observation was significantly lower than that at the beginning of the observation[(2.05±1.70)kU/L vs(16.50±9.87)kU/L,t=11.04,P<0.01].The multiples of the value of serum IgE at the end of the observation was significantly lower than that at the beginning of the observation[(4.50±1.08)times vs(11.99±5.35)times,t=10.51,P<0.01].Conclusion:The symptoms of patients with severe eczema induced by milk allergy can alleviate after the long period of milk avoidance with decreasing levels of milk-specific IgE and serum IgE.
