摘要
目的检索、评价和总结造口患者围手术期健康教育的相关证据,为临床提供参考。方法检索加拿大安大略注册护士协会网站、苏格兰学院间指南网、新西兰指南协作组、澳大利亚国立健康与医学研究理事会网站、国际指南协作网、BMJ最佳临床实践、UpToDate、乔安娜布里格斯研究所循证卫生保健中心数据库、Cochrane Library、PubMed、万方数据库、中国知网等指南网及数据库,以及世界造口治疗师协会、美国伤口造口失禁护士协会、美国结直肠外科医师协会等专业网站中有关造口患者围手术期健康教育的相关证据,包括循证指南、系统评价、推荐实践和证据总结,检索时限为建库至2020年1月7日。由2名研究者对文献质量进行评价和资料提取。结果共纳入11篇文献,其中指南5篇、系统评价2篇、推荐实践1篇和证据总结3篇。最终总结了18条最佳证据,分别为教育者资质、施教对象、教育时机、教育前评估、术前教育、术后教育、教育策略及效果评价。结论最佳证据显示,合理进行造口患者围手术期健康教育,可降低术后并发症发生率,提高患者的生活质量,促进其早日回归社会。
Objective To retrieve,evaluate and summarize the best evidence of perioperative health education for patients undergoing ostomy surgery. Methods We searched Registered Nurses’ Association of Ontario,Scottish Intercollegiate Guidelines Network,New Zealand Guidelines Group,Australian Government National Health and Medical Research Council,Guidelines International Network,BMJ Best Practice,UpToDate,Joanna Briggs Institute library,Cochrane library,and special websites,such as World Council of Enterostomal Therapists,Wound,Ostomy and Continence Society,American Society of Colon and Rectal Surgeons,Nurses Specialized in Wound,Ostomy and Continence Canada,United Ostomy Associations of America,American Urological Association and European Associations of Urology,to collect relevant guidelines,evidence summaries,recommended practices and systematic reviews and expert consensuses published from database construction to January 7st of 2020. 2 researchers evaluated the quality of the literature,and extracted recommendations and research conclusions from the included literature. Results Totally 11 articles were involved,including 5 guidelines,3 evidence summaries,1 recommended practice and 2 systematic reviews. Finally,18 pieces of best evidence about educator qualification,education targets,education timing,assessment,preoperative education,postoperative education,education strategy and evaluation were summarized.Conclusion This study summarized the best evidence of perioperative health education for patients undergoing ostomy surgery. With reasonable planning,it can reduce the incidence of stomal and peristomal complications,promote patients to return to society as soon as possible,and thus improve the quality of life of patients.
