摘要
目的以朱丹溪医案作为切入点,采用扎根理论和聚类分析对医案进行处理,挖掘朱丹溪四君子汤应用特色,指导临床上对于四君子汤的应用。方法从中医典籍中收集朱丹溪应用四君子汤的成功医案,对于病名、病因病机、症状采用扎根理论的研究方法,以三级编码的方式进行整理,提取出病因病机。方药部分采用聚类分析,提取四君子汤的配伍特点。结果质性研究结果显示,朱丹溪四君子汤应用的病机主要分为内伤病机、气病机、血病机、外感病机、阴阳病机和脏腑病机六类;聚类结果表明,朱丹溪四君子汤配伍药物主要分为五类,其功效分别是养血活血、理气化痰、补气升阳、清热降火和燥湿化痰。结论四君子汤治气,气虚者可补一身之气,达到养血和温阳的目的,气实者可防火热伤气;四君子汤可健脾以化痰;四君子汤可用于伤寒的治疗和预后调理;四君子汤可防肝木乘脾土。
Objective Taking ZHU Danxi’s medical cases as the starting point,using grounded theory and cluster analysis to analyze the medical cases,to mine the characteristics in use of ZHU Danxi’s Sijunzi Decoction(四君子汤),and to guide the clinical application of Sijunzi Decoction. Methods This study collects all the ZHU Danxi’s cases of successful treatment with Sijunzi Decoction from the traditional Chinese medicine books. For the name of disease,etiology,pathogenesis and symptoms,the grounded theory were used to research. And the etiology and pathogenesis were extracted by the three-level coding. In the part of prescription,cluster analysis was applied to put forward the compatibility characteristics of Sijunzi Decoction. Results The results of qualitative research showed that the pathogenesis of ZHU Danxi’s Sijunzi Decoction was mainly divided into six categories:pathogenesis of internal injury,Qi pathogenesis,blood pathogenesis,exogenous pathogenesis,Yin-Yang pathogenesis and Zang-Fu pathogenesis. The results of cluster analysis showed that the compatibility drugs of ZHU danxi’s Sijunzi Decoction were mainly divided into five categories,with the functions of nourishing blood and promoting blood circulation,or regulating Qi-flowing for eliminating phlegm,or invigorating Qi for promoting Yang,or clearing heat and fire or drying dampness for resolving phlegm. Conclusion Sijunzi Decoction could treat the Qi disease. For the persons have symptoms of Qi deficiency,it can tonify Qi of the whole body to achieve the purpose of nourishing blood and warming Yang. For the persons have symptoms of hyperactive Qi,it can prevent heat from damaging Qi. Sijunzi Decoction can invigorate spleen to remove phlegm and can be used for the treatment and prognosis of typhoid fever. Otherwise,Sijunzi Decoction can prevent liver from over-restraining spleen.
